Description: | This fellowship training opportunity aims to provide physicians with specialized clinical training in providing high-quality palliative and supportive care for adolescents and young adults (AYA), aged 15 to 39 years, with advanced cancer. Existing evidence suggests that AYAs with cancer have unique psychosocial (removed comma) and palliative care needs that differ from those of pediatric and older adult patients. Fellows will gain competence in providing specialized palliative care for young adults in different palliative care practice settings, such as an inpatient tertiary palliative care unit, inpatient palliative care consultation service, and specialized outpatient AYA palliative care clinics, the latter of which include direct collaboration with an AYA psychiatrist. Additionally, fellows will have the opportunity to develop research proposals under supervision, analyze original research data, prepare presentations for local, national and international meetings, and write manuscripts for submission to peer-reviewed journals. 30% of the fellowship time is protected for research and other scholarly activities. Fellows can advance their leadership skills through the leadership opportunities provided during the training. Eligible applicants to this fellowship are required to have completed clinical training in palliative medicine (Royal College training or equivalent). |

Description: | The purpose of the Palliative Medicine (and Advanced Care Planning) Clinical fellowship is to train physicians to be competent in practicing palliative care at a consultant level. Fellows will have outstanding clinical experience and will work in different palliative care practice settings across Toronto, such as community and tertiary palliative care units, inpatient palliative care consultation service, residential hospice, and outpatient palliative care clinics. Fellows will learn about palliative care for patients and families at diagnosis, during active treatment, in the event of acute medical issues, after completing treatment, and at end-of-life. Fellows will have the opportunity to observe and perform palliative care consultations with supervision to aid in developing diagnostic and formulation skills, and will receive ongoing clinical supervision. During the training, fellows will provide palliative care for a wide spectrum of patients with malignant and non-malignant illnesses. There is also the opportunity to tailor training to individual learning goals. Clinical fellows will complete a scholarly project during their training and will participate in an inter-disciplinary academic environment with ample opportunity for feedback and presentation at local rounds, including Pain and Symptom Management Rounds and Departmental Grand Rounds as well as leadership opportunities, including junior attending roles and committee participation. |

Description: | The Palliative Medicine Clinical Research Fellowship at the University Health Network provides fellows with an outstanding experience in clinical palliative care research. The focus of the fellowship is on acquiring research skills while working in an interdisciplinary clinical and academic environment. Fellows will have the opportunity to develop research proposals under supervision, analyze original research data, prepare presentations for local, national and international meetings, and write manuscripts for submission to peer-reviewed journals. There is ample opportunity for presentation at local rounds, including the Manuscript Review Seminar, Journal Club, Pain and Symptom Management Rounds and Departmental Grand Rounds. Concomitant completion of a Master's degree with a focus on palliative care is also possible. Fellowships are for a 2-year duration, with the second year contingent on a successful evaluation after one year; there is 60% protected time for research, with the remaining time spent mainly in specialized outpatient palliative care. Eligible applicants to the Clinical Research Fellowship are required to have completed clinical training in palliative medicine (Royal College training or equivalent), as the training emphasis is on developing research skills. |

Description: | This fellowship is an educational opportunity to learn more about amputee management including pre-amputation consults, acute care consults post-amputation, inpatient and outpatient rehabilitation. An outpatient fellow clinic will be developed. The fellow will be expected to attend family meetings, make clinical decisions regarding prosthetic care with team members, deal with both standard prosthesis as well as the state of art advances in amputee care. Fellows will also be engaged in medical residents and medical students teaching. The Sunnybrook amputee population consists of dysvascular patients as well as trauma, burns and cancer patients. The Sunnybrook campus in addition to being a fully affiliated University of Toronto teaching hospital also hosts George Brown College Prosthetist training diploma program. As the largest acute care trauma hospital, this Sunnybrook fellowship offers exposure to complex traumatic and burns related amputations both upper and lower extremity. |

Description: | The fellowship is an educational opportunity to learn about brain injury medicine rehabilitation. The fellowship experience will include working on two inpatient acquired brain injury (ABI) units as well as outpatient services with extensive exposure to acquired and traumatic brain injuries (ABI/TBI). The pathologies of the brain injuries are mainly due to infections, vascular, tumour/cancers, and poly-trauma (pedestrian and motor vehicle). Fellows will gain significant clinical experience in managing of adult brain injury and botulinum toxin injections for spasticity/hypertonicity. |

Description: | Toronto Rehabilitation Institute, a member of the University Health Network, has a long track record of providing specialized outpatient, interdisciplinary team-based rehabilitation to persons living with Multiple Sclerosis (MS) with mild to moderate levels of impairment. Embedded alongside this program, there exists a large neurophysiatric practice catering to the bio-psycho-social needs of MS patients at all levels of disability. Furthermore, strong collaborations exist with Unity Health--St. Michael’s Hospital’s BarLo MS Centre, a world- leading, neurology-led, MS clinic that also has a neurophysiatric presence. This dual setting, is ideal for the physiatrist or neurologist post-graduate learner interested in increasing their knowledge of the holistic management of persons living with MS to minimize disability progression, optimize functional independence, and decrease symptom burden. Motivated, caring and dependable specialist physicians would be most welcome to become part of a team dedicated to the well-being of persons living with MS. Participation will enhance one’s knowledge of MS specific impairments (including but not limited neurogenic fatigue, spasticity, MS-related sleep disorders, MS-related psychiatric presentations, neurogenic bladder, bone health, etc.) and physiatric principles, as a whole. State of the art information on MS disease modifying treatments, disease surveillance, exercise, and other wellness topics to address physical, cognitive, and psycho-emotional wellbeing will be discussed throughout the course of the fellowship. Opportunities to collaborate on research will be available to all clinical fellows expressing an interest. This exclusively outpatient-based practice would complement the acquisition of knowledge, skills, and attitudes relevant to the rehabilitation of other brain and spinal cord-based pathologies and may form part of a larger neuro-based rehabilitation fellowship for those candidates interested in broader exposure. |
