Description: | Obstetric Medicine is a subspecialty of Internal Medicine that focuses on the management of medical disorders in pregnant women. This discipline aims to optimize the medical issues in women prior to pregnancy, during pregnancy and in the postpartum period. In this fellowship, trainees will rotate through a side variety of subspecialty clinics that focus purely on the pregnant patient. These include endocrinology, hematology, cardiology, neurology, and nephrology. In addition, fellows do inpatient consults on the antenatal and labour and delivery ward. Fellows will also acquire the knowledge for the safe prescribing of drugs in pregnancy and lactation. This fellowship has been available at the University of Toronto for 15 years, and is open to physicians with a background in General Internal Medicine as well as to subspecialists who wish to acquire a broad skill set in the management of a wide variety of medical disorders, such as hypertension, preeclampsia, diabetes, cardiac disease and renal disease. Fellows will be involved in case conferences, teaching rounds, and will also be required to supervise more junior trainees. Research is encouraged, although is not a mandatory requirement of the training program. |

Description: | This is an opportunity to join one of the very few Hospitalist Medicine fellowship programs in North America and the first in Canada. Its curriculum has been specifically designed to provide trainees with the opportunity to expand their expertise in hospital-based medical care, and particularly focuses on the care of the acutely ill medical patient and on hospital systems of care. It is anticipated that by the end of the program year that candidates will be capable of delivering comprehensive medical care to virtually all hospitalized patients including managing critically ill patients and providing consultation services to the Emergency Department and to surgical services. In addition to assisting physicians to improve their clinical knowledge and skills through exposure to a multitude of medical conditions, the program particularly emphasizes training in medical education, quality improvement methodologies, clinical ethics, and hospital systems and their relationship to medical practice. Our curriculum is based on the Society of Hospital Medicine core competencies. Fellows work as core members of a variety of clinical teams providing comprehensive hospital-based care to medical patients, including emergency evaluation and management, critical care, general medical care, and transitional care; a committed group of University of Toronto faculty members supervise and manage the program and are closely involved in the education of Fellows. This remarkable educational experience is further complemented by a close working relationship with faculty members of virtually all medical and surgical subspecialties. |

Description: | The UHN/SHS Clinical Fellowship in Hospital Medicine is suitable for physicians who have completed training in Family Medicine or Internal Medicine, and who wish to acquire additional expertise and experience in managing complex medical inpatients. Fellows will gain competencies in areas such as management of common acute medical conditions, bedside procedural skills, health system navigation, quality improvement and patient safety, communication skills and teamwork. In addition to a focus on complex medical inpatients, our program offers the unique opportunity to work with specialized patient populations such as cancer, perioperative and psychiatry inpatients. This allows our graduates to meet the demands of the growing roles of Canadian hospitalists in co-managing hospitalized patients. Our Program offers a 1-year fellowship, based at the Toronto General, Toronto Western and Mount Sinai Hospitals. Under exceptional circumstances and at the discretion of the Fellowship Trainee Committee, contract extensions up to an additional year can be offered. As of July, 2024 there are 16 available positions per year. Fellowships start on July 1, with some flexibility depending on the availability of vacant positions. This position is suitable for residents interested in doing full-time or part-time hospitalist work, rural and/or suburban locums managing acute medical inpatients and those who are looking for further enhanced skills in Hospital Medicine. The interface working with specific patient populations (SHS – Orthopaedics co-management, UHN – Oncology) allows for graduates of our program to leave with unique skill sets that confer greater marketability. Clinical Fellows rotate through each of the Hospital Medicine Service sites, working closely with the interprofessional team and supervising faculty. As Clinical Fellows gain more knowledge and experience, they are also given increasing responsibility. In addition to working weekdays, Fellows are responsible for in-house call whereby they are responsible for inpatient coverage and ER admissions with 4-5 calls (2-3 weekday nights and 2 weekend 24-hour coverage shifts). Clinical training is centered on daily patient care activities, including reviewing all new admissions, daily discussions around the ongoing management of patients on the service, and comprehensive discharge planning involving all trainees, supervising faculty, and the allied healthcare professionals on the team. The Hospital Medicine team is supervised by a faculty member from the Division of General Internal Medicine. The team includes Hospital Medicine Clinical Fellows, elective residents in Internal Medicine and Family Medicine residents, medical students, and various inter-professional team members. As a Clinical Fellow, you will develop competencies in emergency department consultations, inpatient management of common acute medical conditions, perioperative management, discharge and transitions planning, teamwork, quality improvement, and bedside procedural skills. OVERVIEW OF THE FELLOWSHIP: The Unity Health Toronto Hospitalist Fellowship Program is a unique 12-month competitive training opportunity for both Canadian and International physicians who are graduates of a residency program in Family Medicine or Internal Medicine, to acquire advanced skills in the management of complex, hospitalized medical patients. In addition to caring for acutely-ill patients affected by multi-system disease or presenting with undifferentiated problems, excellence and compassion in treating inner city patients will be a focus. This includes the pharmacologic treatment of substance misuse and addictions, advanced discharge planning and mobilization of community supports for those experiencing homelessness, culturally-sensitive care, assessment and management of the frail elderly, and more. Trainees will have access to distinct learning by faculty who are experts in their field and will receive ample protected time and strong faculty mentorship for education, research, quality improvement, and leadership endeavors. Graduating fellows will have the necessary skills, experience, and confidence to practice independently anywhere in the world and will be highly sought after for permanent hospitalist positions in a faced-paced, diverse health care settings that may include urban, community, or rehabilitation sites. Description: | The Sunnybrook General Geriatric Medicine fellowship is a 1-year program. During this program, fellows will rotate through specialized geriatric services at Sunnybrook Health Sciences Centre. Geriatric consultation to geriatric trauma and geriatric burn patients will be the focus. Consultations to other medical and surgical services at the Bayivew campus will also included. Outpatient geriatric clinic, falls clinic will take place either in distributed clinics throughout the year or during blocks, depending on scheduling. Geriatric Day hospital and outreach team assessments will be offered. Description: | The Geriatric Medicine SEAP fellowship is a 2-year program mirroring the University of Toronto Geriatric Medicine subspecialty residency program that adheres to the Royal College of Physicians and Surgeons of Canada Competence By Design Curriculum. Graduates of this SEAP fellowship can apply to write the Royal College exam in Geriatric Medicine and if successful, can apply to become Subspecialty Affiliates of the Royal College. During this program, fellows will rotate across the 4 academic teaching hospitals offering geriatrics training rotations within the city of Toronto (Sunnybrook Hospital, Unity Health, Baycrest Hospital and Sinai Health System/University Health Network Hospitals). Clinical rotations include: - Acute Care of the Elderly (ACE) Unit -Inpatient geriatric consultation -Geriatric rehabilitation -Geriatric psychiatry -Behavioural neurology -Ambulatory (outpatient) geriatrics, including one half-day longitudinal clinic each week. Up to eight elective blocks can be arranged according to the interests and needs for the fellow.The clinical experience is enhanced by dedicated educational activities including: - Weekly academic half day teaching -Scholarly project: with mentorship and protected time during the program to facilitate project completion -Monthly journal club -Quality improvement co-learning curriculum: conduct a group QI project with a faculty liaison. Prior to enrollment, the program will work with the candidate to ensure eligibility to write the Royal College of Physicians and Surgeons of Canada Geriatric Subspecialty Exam at the completion of training.
