Description: | Established in 2006 by Dr. Herbert Ho Ping Kong and funded by G. Raymond Chang, O.C., O.J., this fellowship program supports clinicians from the West Indies (the islands of the former British West Indies) interested in pursuing academic Internal Medicine subspecialty training at UofT. This clinical fellowship is a limited term academic experience, with the objective of providing advanced training and specialized expertise in Internal Medicine subspecialties including, but not limited to Cardiology, Rheumatology, Nephrology, Infectious Diseases, and Gastroenterology or in a closely related discipline such as Neurology. This training is not currently available in the West Indies and such clinical experience is increasingly needed in the tertiary care hospitals around the Caribbean. Upon completion of the fellowship training, physicians will return to their primary institution. The specific goals and objectives would vary depending on the subspecialty.

Description: | The fellowship is focused on the novel cross-disciplinary field of Cardio-Oncology. This fellowship will allow the trainee to learn the shared risk factors for cancer and cardiovascular disease, the potential complications associated with cancer therapeutics, methods of prevention, early detection, and treatment. There will be a significant focus on clinical research. The fellowship will predominately focus on outpatient care. Description: | The Adult Echocardiography fellowship provides formal training in advanced echocardiography (transesophageal, stress (exercise/dobutamine), three-dimensional echocardiography, strain imaging, intraprocedural echocardiography). The Echocardiography Laboratory at the University Health Network is a high-volume center (>25000 cases per year) and a quartenary care facility with expertise in complex cardiac conditions (eg. advanced heart failure and cardiomyopathies, valvular heart lesions, adult congenital heart disease). Research opportunities during the fellowship period include investigations involving novel echocardiographic techniques, the role of echocardiography in the management of cardiomyopathies and valvular lesions, and assessment of educational and quality methods in echocardiography. Fellows will be expected to be involved in all aspects of arrhythmia-related procedures.
It is not expected that fellows will perform all the above tasks for every patient. However, some cases should be followed through in all aspects from start to finish, while involvement with other cases will only be partial. The clinical care of arrhythmia patients will be shared between the EP fellow and staff physicians, but it is important for fellows to be aware that arrhythmia training extends outside the EP lab or OR. Fellows will be expected to scrub in 150-200 ICD/Pacing/EP cases per year. In accordance with the University of Toronto Division of Cardiology policy, EP fellow (along with other sub-specialty fellowship trainees) may be asked to cover general cardiology in-house call for up to two nights per month. It is anticipated that clinical responsibilities will comprise about 50-60% of the fellows’ time.
RESEARCH The research experience in this combined clinical and research fellowship will be viewed as an essential component, with up to 40% of time allotted to research activities. Both expectations of, and support for, research productivity can be anticipated. Early in the fellowship (within the first four weeks), fellows should conceive (with input from staff) at least one independent research project which they will design and carry out over the year. Submission of one or more abstracts to CCS and AHA (in the spring) will be strongly encouraged.
In the interest of completing the project, the goals should be realistic, and the project should take advantage of existing expertise and infrastructure. Fellows must also be familiar with ongoing EP research protocols and should help in recruitment of patients to clinical trials. Depending on interest and career objectives, the fellow should also consider becoming involved as a collaborator or research assistant on an additional, established project. ROUNDS AND TEACHING Fellows will be responsible in presenting cases at Cardiology non-invasive and Grand Rounds. At least one presentation will be expected during the year at either the Research Rounds (0800 Thursday). On a very occasional basis, fellows may be asked to conduct teaching rounds with the housestaff in the CCU or on the cardiology ward. Didactic teaching will be provided to interventional fellows in accordance with 1999 guidelines of the American College of Cardiology. It is anticipated that these sessions will be run on a “city-wide” basis and include trainees from the other University of Toronto hospitals which have EP services. REMUNERATION There are no funds provided by the University, the hospital, or the Department of Medicine for fellows’ salaries. Prospective fellows are strongly urged to pursue independent sources of funding. However, the Division of Cardiology has agreed to support the specialized training of highly qualified candidates. The amount of support provided will be negotiated, and will depend on the amount of alternate funding available. Fellows will be provided with a pager, lab coats and scrub suits, a comfortable workspace (shared office), and access to a computer (with e-mail and internet access). Financial support to attend at least one major conference per year will also be provided. Vacation allowance consists of three weeks per year. EVALUATION Fellows will meet with the Medical Director of Arrhythmia Service every three months, with both parties providing oral feedback. A written evaluation (ITER) will be completed after 6 and 12 months. On completion of the fellowship program, a certificate will be issued by the Hospital and the University. In addition to scheduled evaluations, ongoing, informal, bi-directional evaluation is strongly encouraged. |

Description: | The Schulich Heart Program is pleased to offer a one-year multi-modality clinical imaging fellowship. The major focus of fellow will be echocardiography, with the goal of achieving a high level of clinical expertise in all applications of this modality. Qualification for Level III certification in Echocardiography, and completion of the National Board of Echocardiography examination, which takes place in July each year, are worthy goals. In addition there will be exposure to cardiac MRI and CT. Primary supervision of the imaging fellow will be by Drs. Joyner, Ahmed, Hansen, Poon and Roifman (echocardiography) and Dr. Anna Zavodni- Medical Imaging (MRI and CT). Drs. Gilbert, Morgan, Poon and Lau will also contribute to supervision of echocardiography. Primary supervision of research will be by Dr. Graham Wright. Clinical Responsibilities: The fellow will be expected to be involved in all aspects of cardiac imaging-related procedures, including:
It is not expected that fellow be involved with every task for every patient. However, it is the joint responsibility of the attending staff, imaging technologists, and fellow to ensure that educational opportunities involving interesting, rare, or unusual cases not be missed. Fellows will be expected to be involved with interpretation and/or scanning of a large volume of echo studies daily, including all interesting/rare/unusual cases that pass through the lab. The fellow will be expected to take part regularly in supervision and interpretation of exercise stress echo. The fellow should be involved in most TEEs that pass through the lab. It is expected that the imaging fellow may be asked to help occasionally with clinical care of CCU or Cardiology ward patients. The fellow may be asked to participate in a TEE on call schedule shared with Anesthesia. No other call responsibility is mandated, although the option of after-hours involvement with interesting cases is encouraged if it is felt to be of genuine educational value, and not an onerous burden. Research: There is the option of being involved in a research project depending on the needs and interest of the fellow. The supervisor for this component of the fellowship is Dr. Graham Wright, research director for the Schulich Heart Program. There is the potential to spend one day per week on research. Teaching is a key mandate of the Sunnybrook non-invasive lab, and an important component of the cardiac MRI and CT programs. The fellow is expected both to benefit from, and contribute to, an excellent teaching environment. Cooperation with core cardiology trainees, elective trainees, and the other Imaging Fellow– especially in situations of limited educational resources – is essential in maintaining a productive atmosphere. Teaching of cardiology residents is an important mission of the non-invasive cardiac laboratories and the fellow is expected to assist in this process. “To teach is to learn” – and it is hoped that the fellow will contribute to this mission. In return, the fellow will receive regular, formal and informal teaching in all aspects of clinical imaging and in conjunction with research efforts. The fellow will be responsible for presenting and leading non-invasive imaging rounds on a regular basis. Presentation at cardiology grand rounds and cardiology research rounds is strongly encouraged. Description: | The Cardiac Prevention fellowship program at St. Michael's Hospital is a 12-24 month fellowship providing training in all the essential aspects of contemporary preventive cardiology with an emphasis on risk reduction strategies in the patient with coronary disease. Ambulatory care will be emphasised. As well, there is the opportunitiy to explore postmenopausal women's health and aspects of gender equtity. By the end of the training period, it is expected that trainees will have acquired the knowledge, experience, and clinical and technical skills required to be an expert in the diagnosis and management of the entire spectrum of cardiac prevention. GIM training is a prerequisite. The fellowship will be tailored to the trainees background (GIM vs Cardiology training) and needs. In addition to clinical expertise, trainees will also have the opportunity to pursue academic or scholarly activity in the area of cardiac prevention with appropriate supervision at a level required to further a career in academic cardiology, GIM, and/or cardiac prevention. |

Description: | The Floras laboratory (Clinical Cardiovascular Physiology Laboratory at Toronto General Hospital) is recognized internationally for discoveries arising from its investigation of circulatory and cardiac control mechanisms in human health and disease. Its,particular emphasis is the role of the autonomic nervous system in heart failure, hypertension, renal failure, and sleep-related breathing disorders. Human research methods include microneurographic recording of post-ganglionic sympathetic nerve activity, frequency domain analysis of cardiovascular signals, cardiac and brachial ultrasound imaging and functional magnetic resonance brain imaging, Research findings appear in over 200 original publications and also are summarized and contextualized within a range of invited reviews, editorial commentaries, and book chapters. All are accessible through PubMed or via the web sites of the Toronto General Research Institute or the Lunenfeld-Tanenbaum Research Institute, Mount Sinai Hospital. Looking forward, there is latitude to develop innovative projects in collaboration with clinician scientists in the disciplines of respiratory and sleep medicine, nephrology, coagulation, diabetes, women’s cardiovascular health, pregnancy, and exercise science plus opportunity to lead new initiatives involving device-based strategies for restoration of autonomic balance in heart failure. |

Description: | The Adult Interventional Cardiology program at the University Health Network is committed to offering fellows a comprehensive training program in the discipline of percutaneous coronary intervention and interventional cardiology. Fellows will spend the majority of their clinical experience assigned to the cardiac catheterization laboratory. Fellows will be expected to be involved in all aspects of percutaneous coronary procedures. Prior to a procedure, the fellow sees and examines the patient, explains the procedure to be performed and obtains informed consent. Fellows will be expected to scrub in on >500 cases per year of which >250 it will be expected that they serve as the primary operator of an interventional procedure. The case work will include conventional balloon angioplasty, a wide variety of stents, rotational atherectomy, possibly directional atherectomy, thrombectomy, as well as ancillary diagnostic and therapeutic tools such as intra-vascular ultrasound, and intra-coronary Doppler and/or pressure measurements and/or OCT. In addition, the fellow will gain experience and competency with insertion and management of ventricular assist devices. The research experience is viewed as an essential component of the fellowship training program. Up to 40% of fellowship time (over two year training program) may be allocated to research activities, provided reasonable expectations that clinical experience and portfolio competencies will concurrently be met.Nonetheless, both expectations of, and support for, research productivity should be anticipated. Early in the fellowship (within the first eight weeks), the site program directors will facilitate a connection between the interventional fellows and potential faculty research mentors; in this fashion, fellows can maximize their academic productivity during their two-year program. They should conceive of (with input from a staff mentor) at least one independent research project, which they will design and carry out the full scope of project involvement (enrollment of patients, data analysis, presentation or publication of results including in abstract form and complete manuscripts) over the course of their fellowship. The submission of at least one abstract to the Canadian Cardiovascular Congress or another major Interventional meeting OR one journal article accepted or published will be expected over the duration of the two year fellowship program. In the interest of completing the project, the goals should be realistic, and the project should take advantage of existing expertise and infrastructure. Fellows must also be familiar with ongoing interventional research protocols and should help in recruitment of patients to clinical trials. Depending on interest and career objectives, the fellow can also consider becoming involved as a collaborator or research assistant on additional established projects In accordance with the University of Toronto Division of Cardiology policy, Fellows may be asked to cover in-patient general cardiology ward service up to 2 nights per month. Interventional Cardiology fellows are also expected to participate in the Interventional cardiology call schedule on a rotating basis, which is done as home call. |

Description: | CLINICAL RESPONSIBILITIES
It is not expected that fellows will perform all the above tasks for every patient. However, some cases should be followed through in all aspects from start to finish, while involvement with other cases will only be partial. The clinical care of angioplasty patients will be shared between the interventional fellows and staff physicians, but it is important for fellows to be aware that angioplasty training extends outside the cath lab. Pre-assessment and follow-up clinics will be arranged in blocks so that fellows can see several patients during a half-day session. Approximately one such session per week will be scheduled. Fellows will be expected to scrub in on 300 to 400 cases per year. Based on experience and case complexity, they will progress from second to first operator to solo first operator (with staff physician in the control room). The case work will include conventional balloon angioplasty, a wide variety of stents, rotational atherectomy, possibly directional atherectomy, thrombectomy, as well as ancillary diagnostic tools such as intra-vascular ultrasound, and intra-coronary Doppler and/or pressure measurements. A good angioplasty operator must be capable of performing very high quality diagnostic angiography. Therefore, fellows will perform at least 50 diagnostic studies under the supervision of a staff physician, mostly concentrated in the early months of training. Subsequently, fellows may be asked on occasion to perform a limited number of diagnostic studies. In accordance with University of Toronto Division of Cardiology policy, interventional fellows (along with other sub-specialty fellowship trainees) may be asked to cover general cardiology in-house call up to two nights per month. It is anticipated that clinical responsibilities will comprise about 50-60% of the fellows’ time. RESEARCH ROUNDS AND TEACHING Interactive small group teaching will be provided to interventional fellows through the University of Toronto monthly ‘City-wide’ Interventional Cardiology Fellows Teaching series. This highly regarded longitudinal teaching series is unique amongst interventional training programs in Canada. It covers major topics in interventional cardiology and draws from the expertise of the teaching staff at all three University of Toronto interventional sites (SHSC, SMH, UHN). |

The group has produced important research on the impact of pregnancy on women with heart disease. We offer 1- and 2-year fellowships. The focus of the fellowship is to gain clinical and research expertise in pregnancy and heart disease. Trainning includes understanding pregnancy risks in this population including an indepth knowledge on the impact of pregnancy on the diseased heart, risk stratification of the pregnant women with heart disease, assessment of the safety of medication use during pregnancy, management of complications during pregnancy, at labour and delivery, and postpartum. Fellows are expected to participate in teaching cardiology residents, internal medicine residents and medical students both while in clinic and also in formalized teaching roles (sub- specialty rounds and teaching sessions organized through the post-graduate medical education and undergraduate medical education programs at UofT). Fellows are expected to take an active role in the cardiovascular outcomes research program, a critical research initiative at WCH and UofT. The percent of protected time for research and educational activities will vary between applicants but is expected to be approximately 75%. Fellows will also have an opportunity to lead a secondary analysis of a completed trial or observational/registry study with the intent that the manuscript is submitted for publication in a peer-reviewed journal by the end of their term. |

Description: | The fellow will develop expertise in 3 core areas: cutaneous malignancies, skin toxicities of anti-cancer treatments, and cutaneous medical oncology. The program will include the full spectrum of clinical management, from clinical diagnosis to histopathological confirmation and surgery and chemotherapeutic treatments for a wide range of cutaneous malignancies including melanoma, high-risk basal and squamous cell cancers, and rare skin cancers such as Merkel cell carcinoma and other cutaneous malignancies. Description: | The Laser and Cosmetic Dermatology fellowship is designed to allow the fellow to become an expert in cutaneous laser and energy-based devices, photodynamic therapy and cosmetic dermatology (e.g., soft tissue fillers, neuromodulators, chemical peels, and sclerotherapy). The fellow will develop skills communicating treatment options and plans, risks and benefits of treatment, and realistic expected outcomes. Description: | The inflammatory skin disease fellowship at University of Toronto is a 12-month multi-disciplinary program, incorporating general dermatology and internal medicine. The fellowship is a comprehensive training program that aims to prepare clinician-researchers to care for the wide spectrum of complex dermatological cases encountered in a teaching hospital. Fellows learn to manage inflammatory skin disorders such psoriasis, atopic dermatitis, hidradenitis suppurativa (HS) as well as less common and complex wounds due to systemic disorders such as pyoderma gangrenosum (PG). The Fellowship also fosters a rich environment for productive research activities where fellows are encouraged to develop specific research interests. The Fellowship provides exposure to all aspects of research including clinical research (investigator initiated and industry sponsored trials) as well as robust basic science laboratories. Fellows learn hands on topics in study design, regulatory and ethical considerations as well as “real-life” execution of research protocols. When and how do people become fellows? Registration for candidates generally begins 6 months before the anticipated start date; interviews are conducted in the fall for the upcoming year. Our training program is one year, generally running from July 1st to June 30th, however the timelines are flexible. Typically, the CV, letter of interest and three letters of reference are needed for the application. Further documents, required for registration, will be requested if the application is successful. Who can apply? Candidates who completed a full dermatology residency by the time the fellowship commences will be considered. International candidates (i.e., outside the USA or Canada) are welcome. What is our fellowship like? The fellowship is based at Women’s College Hospital, affiliated with the University of Toronto. We expect the fellow will attend clinics with faculty involved in clinical and research work in inflammatory skin diseases, to advance their knowledge and skills in these areas. The program also offers additional opportunities to develop interest in basic science research and clinical research. The fellow is expected to complete a fellowship research project, as per University of Toronto guidelines. The fellow will spend approximately 50% of their time in outpatient clinics (clinical time commitment will depend on research productivity), where they will learn, in a highly mentored environment, the fundamentals of inflammatory skin disease treatment and prevention. The rest of the time will be dedicated to scholarly activities (research and education). The fellows will teach trainees informally at the bedside and also 1-3 hours of formal lectures each month for residents and students. The objectives of the fellowship-training program are:

Description: | Ornge, a not-for-profit organization, is Ontario’s provider of air ambulance and related services. Established in 2006, Ornge is responsible for approximately 21,000 patient-related transports each year, serving a population of more than 13 million people across 1 million square kilometres. With a fleet that includes airplanes, helicopters and land ambulances operating from 12 bases across the province, Ornge crews perform emergent and urgent inter-facility transports and emergent on-scene responses while supporting healthcare in remote communities across Ontario. The Ornge team consists of paramedics, pilots, aircraft maintenance engineers, communications officers, physicians, educators and support staff. The Fellowship will provide a unique experience specifically focused on air medical and land critical care transport. The Fellowship will include an immersive clinical experience in critical care transport in both an urban helicopter emergency medical services environment and fixed-wing opportunities in resource-challenged settings in rural and remote locations throughout northern Ontario. The hospital-based experiences may also include trauma team leader experience at Canada’s busiest trauma centres in Toronto. |

Description: | The fellowship is intended to develop an Endocrinology trainee’s skills as an endocrinologist using innovative technologies in the management of type 1 diabetes. More specifically, this fellowship will enable the fellow to develop an expertise in advanced care in diabetes, including insulin pump therapy, continuous glucose monitoring, and the implementation of hybrid closed loop algorithm therapy. Description: | St. Michael’s Hospital has a long tradition (over 30 years) of excellence and training in therapeutic endoscopy and remains the largest referral center in the country. The fellowship represents a comprehensive advanced endoscopy experience designed for those interested in an academic medicine career. Fellows will work with the 6 advanced endoscopists (Drs. Marcon, Kortan, Kandel, May, Teshima and Mosko) as well as other members of the GI division at SMH. They will gain extensive experience in a broad range of therapeutic techniques including: ERCP (including cholangioscopy), diagnostic and therapeutic EUS, pancreatic fluid collection drainage, UGI endoscopic mucosal resection, large colonic polyp resection, luminal stenting, radiofrequency ablation and double balloon enteroscopy. In addition to the cognitive and technical skills needed to provide clinical care in interventional endoscopy, fellows will be provided with research mentoring and will be expected to conduct meaningful research, present their findings at international meetings and publish in peer-reviewed journals. |

Description: | Our center performs approximately 180 adult liver transplants per year, of which approximately 20-25% are living donors. We follow >1700 recipients in clinic, and maintain a waiting list of 200-220 patients. Approximately 10-15 new patients are assessed for transplant weekly. We have four funded Tx hepatology fellowship positions. The trainees come for one or two years; one year is typical if their plan is to return and work in a peripheral center that works with, but does not perform, transplants, and two years if full employment in a transplant center is the ultimate goal. Our fellows are paid a salary of approximately 65,000CDN, for which they do in-house Multi-Organ Transplant Call 3 or 4 times per month. The fellows rotate a week at a time (depending on numbers) on inpatient, outpatient, and research. There is an opportunity for some endoscopy. Two year fellows have the option of arranging selective in areas such as HCC, radiology, pathology, ID, and others. |

Description: | Obstetric Medicine is a subspecialty of Internal Medicine that focuses on the management of medical disorders in pregnant women. This discipline aims to optimize the medical issues in women prior to pregnancy, during pregnancy and in the postpartum period. In this fellowship, trainees will rotate through a side variety of subspecialty clinics that focus purely on the pregnant patient. These include endocrinology, hematology, cardiology, neurology, and nephrology. In addition, fellows do inpatient consults on the antenatal and labour and delivery ward. Fellows will also acquire the knowledge for the safe prescribing of drugs in pregnancy and lactation. This fellowship has been available at the University of Toronto for 15 years, and is open to physicians with a background in General Internal Medicine as well as to subspecialists who wish to acquire a broad skill set in the management of a wide variety of medical disorders, such as hypertension, preeclampsia, diabetes, cardiac disease and renal disease. Fellows will be involved in case conferences, teaching rounds, and will also be required to supervise more junior trainees. Research is encouraged, although is not a mandatory requirement of the training program. |

Description: | This is an opportunity to join one of the very few Hospitalist Medicine fellowship programs in North America and the first in Canada. Its curriculum has been specifically designed to provide trainees with the opportunity to expand their expertise in hospital-based medical care, and particularly focuses on the care of the acutely ill medical patient and on hospital systems of care. It is anticipated that by the end of the program year that candidates will be capable of delivering comprehensive medical care to virtually all hospitalized patients including managing critically ill patients and providing consultation services to the Emergency Department and to surgical services. In addition to assisting physicians to improve their clinical knowledge and skills through exposure to a multitude of medical conditions, the program particularly emphasizes training in medical education, quality improvement methodologies, clinical ethics, and hospital systems and their relationship to medical practice. Description: | 1-year stream: The UHN/SHS Clinical Fellowship in Hospital Medicine is suitable for physicians who have completed training in Family Medicine or Internal Medicine, and who wish to acquire additional expertise and experience in managing complex medical inpatients. Fellows will gain competencies in areas such as management of common acute medical conditions, bedside procedural skills, health system navigation, quality improvement and patient safety, communication skills and teamwork. In addition to a focus on complex medical inpatients, our program offers the unique opportunity to work with specialized patient populations such as cancer, perioperative and psychiatry inpatients. This allows our graduates to meet the demands of the growing roles of Canadian hospitalists in co-managing hospitalized patients. Clinical Fellows rotate through each of the Hospital Medicine Service sites, working closely with the interprofessional team and supervising faculty. As Clinical Fellows gain more knowledge and experience, they are also given increasing responsibility. In addition to working weekdays, Fellows will participate in weekend rounding call responsibilities. No overnight call responsibilities are required. Clinical training primarily is centered on daily patient care activities, including reviewing all new admission, daily discussions around the ongoing management of patients on the service, and comprehensive discharge planning involving all trainees, supervising faculty, and the allied healthcare professionals on the team. Fellows in this stream will have access to complete up to 1 block of elective during their 1-year training. OVERVIEW OF THE FELLOWSHIP: The Unity Health Toronto Hospitalist Fellowship Program is a unique 12-month competitive training opportunity for both Canadian and International physicians who are graduates of a residency program in Family Medicine or Internal Medicine, to acquire advanced skills in the management of complex, hospitalized medical patients. Description: | The Allotransplant Program at Princess Margaret Cancer Center is the largest in
the Province of Ontario with an annual volume of about 125 procedures that
include grafts from both related, unrelated and haploidentical donors. This
fellowship focuses on clinical management and decision taking for patients
undergoing allogeneic transplantation for various diseases and their early and
long-term management post-transplant. Fellows are expected to participate in
clinical, research and educational activities. They receive a robust exposure to
management of complications such as acute and chronic graft versus host disease,
infectious issues, and emerging co-morbidities such as metabolic disorders and
secondary malignancies. During the fellowship training, they are given the
opportunity to understand the donor search process, apheresis principles and
graft processing including cryopreservation. Knowledge regarding quality control
in stem cell transplantation plays a major role during their training process
and they are exposed to various quality control steps regarding stem cell
procurement, infusion and transplantation process. During the fellowship
training, strong emphasis is given to ensure that trainees at the completion of
his 2 years fellowship program are competent to function as a medical expert for
allogeneic transplantation and health advocate, have acquire appropriate
managerial skills, and communicate and collaborate within their respective
health care environment. Fellows have ample opportunity to participate at
various clinical trials understanding their basic principles and execution. A
formal educational program with regular teaching rounds is in place and
qualifies for CME credits. Candidates for fellowship must have completed subspecialty training in hematology or oncology and be eligible for licensure through College of Physicians and Surgeons of Ontario. |

Description: | This is a unique blended research (25%) and clinical (75% supervised) fellowship between malignant hematology and immune cell therapy. The successful applicant will develop expertise in dealing with patients with hematologic malignancies and all aspects of their care, with a focus on treatment with immune cell therapy interventions including the requirement of bridging therapies and conditioning regimens. The trainee will learn to manage acute complications of immune effector cell therapy; including cytokine release syndrome and immune cellular therapy associated neurotoxicity syndromes. The trainee should have prior experience in allogeneic transplantation, extending these skills to IEC therapy and will become part of a multidisplinary team communicating with other disciplines, e.g. surgical or radiation oncology, psychosocial oncology, in the multidisciplinary treatment of patients. This fellowship will provide experience in managing the rigorous regulatory stipulations for safely conducting cellular therapy clinical trials (FACT and Health Canada accreditation among others).The successful applicant will participate in in-house committees (such as the Immune Effector Cellular Therapy Operations Committee) to recognize different aspects of building an effective program. The trainee will also actively participate in clinical trials, including design and implementation stages.
Majority of their research time will be dedicated to a research project that will be presenting a national/international meeting. At the end of this fellowship, the trainee should have completed at least one peer-reviewed publication. |

Description: | The Princess Margaret Cancer Centre Leukemia program is the largest provider of Leukemia services in Canada, and one of the largest in North America. The program is composed of a multidisciplinary team of physicians, nurses, advanced nurse practitioners, physiotherapists, social workers, and research coordinators dedicated to the treatment of patients with Leukemias and related disorders. Fellows will gain clinical experience in the care of patients with acute leukemia and related disorders. Description: | We are currently offering a clinical research fellowship in the treatment of multiple myeloma and associated mature B-cell lymphoproliferative disorders at the Princess Margaret Cancer Centre (PM). The Princess Margaret Myeloma Group (including six dedicated myeloma/lymphoma physicians) sees approximately 300 new patients and performs autotransplants in over 200 patients per year. Princess Margaret Cancer Centre is the largest myeloma centre in Canada and receives patient referrals from across the country and internationally. The group is actively involved in clinical and laboratory research with international collaborations. Fellows will have the opportunity to gain clinical experience in the care of patients with myeloma and related disorders, including application of the principles of intensive therapy and stem cell transplantation as well as the use of novel agents in drug development. Fellows will have the opportunity to participate in clinical investigation (writing/directing study protocols, participation with the NCIC, applying for grant support, etc). Description: | The Elizabeth and Tony Comper MPN program at Princess Margaret Cancer Center offers a unique opportunity for a clinical or translational fellowship in MPN. Description: | The James Drewry Stewart Fellowship in Multiple Myeloma and Malignant Hematology will provide financial assistance in the form of fellowship grants to oncology trainees at St. Michael's Hospital who have completed their core training in oncology and are seeking additional clinical training in myeloma and other blood cancers. Background: Multiple myeloma and lymphoma are the most common types of blood cancers and the fifth most common cause of cancer overall. Multiple myeloma in particular is a very complex disease that can cause major complications for patients including bone fractures, pain crises, kidney failure and dangerously high blood calcium levels. It is also one of the cancers for which treatment options have advanced the most quickly in the last five years. As a result, expert, patient-centered care for patients with myeloma is essential, and not always readily available. In the community setting, myeloma is frequently managed by medical oncologists rather than by hematologists. Yet, many oncologists receive relatively little training in this specialized cancer. Most oncology training is focused on more common cancers such as breast, colon and lung cancer, and the complexities of myeloma may not be addressed. There is a growing need for advanced clinical training in malignant hematology, and in particular for clinical training in multiple myeloma. |

Description: | The Sinai Health System-University Health Network Antimicrobial Stewardship Program (SHS-UHN ASP) is the oldest and largest ASP in Canada, and has a wealth of experience in teaching antimicrobial stewardship to a variety of health professions, having run national and international antimicrobial stewardship courses. SHS-UHN ASP Team members include physicians, pharmacists, nurses, and project and research support teams. Many members are national leaders in the field. The SHS-UHN ASP Fellowship is a 1-2 year clinical and research fellowship for infectious diseases specialists, medical microbiologists, and general internists planning on leading an antimicrobial stewardship program. Fellows will gain clinical knowledge and experience around important antimicrobial stewardship interventions such as prospective audit-and-feedback, algorithm and guideline development, order sets, digital decision support, and education. Description: | The University Health Network (UHN) and University of Toronto offer a world-class experience for graduating fellows seeking to develop, or continue developing, an academic career in transplant infectious diseases (TID). The program is based on the curriculum suggested by the AST Infectious Diseases Community of Practice and is further enhanced under the leadership and guidance of internationally recognized TID experts: Drs. Shahid Husain, Coleman Rotstein, Atul Humar and Deepali Kumar. Currently, we offer two training tracks for fellows entering our program: a one-year clinical track for fellows interested in a career as a clinician-educator and a two-year track for fellows interested in an academic research career. For inpatient consultation, fellows rotate through two sites, one that specializes in solid organ transplant (SOT) and the other in allo- and auto-stem cell transplant/leukemia/solid tumour. For a comprehensive outpatient experience, fellows have a once-weekly outpatient clinic throughout the fellowship. Fellows can also choose to do electives in primary SOT rotations, which enriches the transplant experience. In addition, we provide a well-structured weekly, interactive lecture series where invited speakers, core faculty, and fellows review the core and advanced topics in TID. Fellows interested in the research track are encouraged to participate actively in the research endeavors of our faculty and begin their own projects. We pay special attention to developing, clinical, translational and laboratory research skills through well-funded clinical and lab-based research programs in TID. Description: | This self-funded fellowship is intended to advance the knowledge and skill set of the trainee in the field of tropical medicine. This is not an introductory fellowship, and is only offered to those with advanced training or experience in the field of tropical medicine, typically signified by some developing world experience and/or graduate training. The fellowship is completed on a part-time basis (2.5-days per week x 1 year for clinical and scholarly work) so as not to impede the fellow's ability to engage in remunerative work that is either hospital or clinic-based. There is no on-call or patient care expectations beyond the time during which the fellow is in clinic with the supervisor. Teaching occurs around each clinical case, and is supplemented by formal and informal lectures, clinical scenarios, readings, and meetings throughout the year. The fellow will also have the opportunity to attend the full roster of educational rounds offered by the Division of Infectious Diseases at UHN, which includes weekly HIV rounds, weekly city-wide ID/Microbiology rounds, weekly clinical case rounds, weekly tropical medicine rounds, and monthly national tropical medicine rounds. The fellow will have the opportunity to engage in a scholarly activity of his or her choosing, and this may include writing of a case report(s) or book chapter(s), or completion of a QI or research project relevant to the practice of tropical medicine. Description: | This fellowship will provide the trainee with the unique opportunity to develop expertise in management of acute events intrinsic to the care of patients with cancer. Advancement in supportive care has over the years increasingly contributed to the improved outcomes observed in most of the cancers. Timely and efficient management of acute events leads to better outcomes, admission avoidance in many cases and reduced length of stay in those patients needing admission. This has led to improved patient centered care, reduced burden on the scarce in-patient beds, and already stressed emergency departments. Acute Oncology is a relatively new specialized field, which is growing in prominence as the complexity of cancer care increases. The trainee will gain significant exposure and adequate hands-on experience in managing these complications associated with cancer care. The trainee will have ample research and teaching opportunities as well as leadership experience. Moreover the trainee will get to work closely with our multidisciplinary team of medical and haematological oncologists, hospitalists, physician assistants, nurse practitioners, nursing, and administrative staff. Description: | The objective of the Princess Margaret Medical Oncology Breast Cancer Fellowship Program is to train early career investigators in breast cancer research and treatment. The Breast Medical Oncology Group at Princess Margaret sees more than 600 new patients referrals annually and interacts with a multi-disciplinary team that includes surgical oncology, radiation oncology, palliative care, pathology, medical imaging, medical genetics, plastic surgery, nursing and social work. Fellows will be mentored by one or more of five full-time staff breast medical oncologists at Princess Margaret with unique areas of expertise and research focus: Dr. Eitan Amir (Health Services & Trial Methodology), Dr. Philippe Bedard (Experimental Therapeutics), Dr. David Cescon (Translational Research), Dr. Christine Elser (Medical Education), and Dr. David Warr (Supportive Care). Fellows will participate in 3-4 half-day ambulatory breast medical oncology clinics weekly under direct staff supervision. Clinical responsibilities will include: new patient consultations, supervision of systemic therapy, toxicity management, and assessment of patients on therapeutic clinical trials. A minimum of fifty-percent of the fellows' time is protected for research. Fellows are expected to develop and implement research projects in their areas of interest in collaboration with their supervisor(s). Description: | The Adult Genetic Epilepsy (AGE) Fellowship Program at Toronto Western Hospital is a well-established program that offers extensive training in all aspects of genetically-determined epilepsies, both common and rare forms. Our AGE Fellowship program is part of one of the largest and most comprehensive Epilepsy Programs in North America, level 4 epilepsy center, with an excellent team of epileptologists, clinical neurophysiologists, genetic counselors, geneticists, neuropsychiatrists, neuroradiologists, neuropsychologists, neurosurgeons, social workers, nurses and dietitians. Fellows will attend the Adult Genetic Epilepsy clinics where they will see a variety of genetic epilepsy cases, including some of the most pharmacoresistant. During this fellowship, fellows will develop expertise in recognition of clinical manifestations as well as indication and interpretation of genetic testing. Our cutting-edge management (both acute and long-term) of genetic epilepsies in adults will give fellows the opportunity to master precision medicine treatments as well as take part in new precision medicine trials. In addition, our multidisciplinary Adult Genetic Epilepsy clinics will give the fellows the opportunity to learn about movement disorders, cognition and other comorbidities seen in adults with genetic epilepsy. The Adult Genetic Epilepsy Program has an important research component and fellows are encouraged to participate in clinical or pre-clinical research. They will have access to the large database and DNA bank of patients with genetic epilepsies and they will be mentored throughout the research. They will also have protected research time. Description: | Clinical Fellowship in Brain Medicine The application deadline for July 2023 start date is FRIDAY FEBRUARY 11, 2022. The Brain Medicine fellowship at the University of Toronto will be offered by the Temerty Faculty of Medicine for a July 2023 start date. This innovative fellowship has been designed to address an emerging need to approach the study of the brain with an interdisciplinary lens and a collaborative approach. The Brain Medicine fellowship will provide successful trainees with an integrated, cross-disciplinary perspective to approach the clinical and research questions that are related to brain diseases. Examples of conditions that lend themselves to an approach facilitated by Brain Medicine include: traumatic brain injury, neurodegenerative diseases, functional neurological disorders, developmental brain diseases (e.g., autism, attention deficit disorder), vascular brain diseases, multiple sclerosis, schizophrenia, and a variety of other neurologic and psychiatric conditions that are presenting with significant disturbances in affect, behaviour, and cognition. The University of Toronto offers the ideal environment for such training given the breadth of expertise represented across brain diseases. The successful fellows will have the opportunity to work alongside internationally renowned mentors and gain experience using cutting-edge clinical and research methods and technologies, including advanced neuroimaging, neuro-interventions, neuromodulation, genetics, fluid biomarkers, advanced clinical trials, and preclinical research. The Brain Medicine fellowship is a competency-based program that aims to provide its fellows with transdisciplinary competencies that complement their postgraduate specialty training; protected time will be provided for academic activities. Candidates must have completed a residency training program in neurology, psychiatry, neurosurgery, neurosurgery, physical medicine and rehabilitation, geriatric medicine, family medicine or other brain related specialty. The Brain Medicine clinical fellowship will focus on the assessment and treatment of impaired affect, behaviour, and cognition (ABC) in complex brain disorders. They will also explore the impact of these disorders on functional abilities, quality of life, and interpersonal relationships. The fellow will gain clinical exposure to multiple complementary disciplines including geriatric medicine, neurology, neurosurgery, physical medicine and rehabilitation, or psychiatry. Description: | The Epilepsy Fellowship Program at Toronto Western Hospital is a well-established program that offers extensive training in all aspects of epilepsy. Fellows will attend clinics with a variety of epilepsy cases, including some of the most pharmacoresistant. Fellows will also receive EEG training, with completion of the Canadian EEG exam expected. Our comprehensive epilepsy program includes neurologists, clinical neurophysiologists, neuropsychiatrists, neuropsychologists, neurosurgeons, social worker, nurses and dietitians. The Program’s strengths include a 10-bed epilepsy monitoring unit, epilepsy genetics clinic, neuromodulation (VNS and DBS), transition from paediatric to adult services, women with epilepsy and epilepsy in pregnancy clinics, specialized ketogenic diet clinic, first seizure clinic and clinical neurophysiology. Fellows will participate in weekly multidisciplinary rounds to discuss patients’ surgical candidacy, weekly EEG teaching rounds, monthly journal club and research rounds. There is a large research program and fellows are encouraged to participate in clinical or pre-clinical research. Research fields include animal models, EEG and MEG, clinical genetics, and pregnancy in epilepsy. Our current faculty members include Dr. Don Weaver, Dr. Richard Wennberg, Dr. Peter Tai, Dr. Aylin Reid, Dr. Martin Del Campo, Dr. Peter Carlen, Dr. Esther Bui, Dr. Eduard Bercovici, and Dr. Danielle Andrade. Term: 1 year with a focus on epilepsy clinics and EEG interpretation. Renewal for a second year of fellowship is an option, depending on availability of funding. Description: | The Movement Disorders Fellowship program at the Toronto Western Hospital is a well-established, popular program, which provides an extensive training in all aspects of clinical movement disorders, including surgical management of patients undergoing functional neurosurgery (see below) and neurophysiology of movement disorders (see below). The training involves attendance at clinics, with faculty members (Drs A Lang, Fox; Chen; Strafella; Marras, Slow; Kalia; Fasano and Munhoz) providing supervision and training. This is a busy unit with the opportunity to see broad and varied clinical cases. In addition there are weekly video-rounds and journal reviews, monthly neuropathology seminars and training in clinical trial design and methodology; as well as the opportunity to attend city wide postgraduate lectures. All fellows also have an opportunity to get training in toxin injections and DBS. There are excellent opportunities for both pre-clinical and clinical research and the fellows are encouraged to purse a research project. The unit collaborates with many world renowned researchers in neurogenetics, basic neuroscience, imaging and neurophysiology. Fellows will get the opportunity to do a rotation in the Tourette and Huntington clinics and all fellows will have the opportunity to spend time in our surgical program clinics. |

Description: | The Multiple Sclerosis Fellowship Program at the St. Michael’s Hospital Barlo MS Centre will train neurologists to offer specialist care to patients with demyelinating diseases of the central nervous system. The duration and design of the training program will be tailored to the specific learning needs and objectives of the candidate, with variable proportions of clinical and research activities. For interested candidates, opportunities may be available to pursue graduate degrees in research or education, while completing the fellowship. Core training will include adult and pediatric out-patient MS clinics and physiatry clinics under the supervision of several different faculty. Description: | Both clinical and research fellowship are available. The fellow is expected to conduct one or more research projects. Our recent projects have examined the pathophysiology of movement disorders (such as Parkinson’s disease, dystonia and tremor), the role of the basal ganglia in voluntary and involuntary movement, and the mechanisms of action of deep brain stimulation. Our laboratory also investigates the organization of cortical inhibitory and excitatory pathways in the human motor cortex and cortical plasticity following peripheral injury such as amputation or nerve and muscle transfer. The fellow will achieve proficiency in advanced methods in human neurophysiology and motor control, including electromyography, transcranial brain stimulation (single pulse, paired pulse and repetitive stimulation), recording and detailed analysis local field potentials from the basal ganglia, computer modeling and MRI co-registration of evoked potentials, electroencephaolgraphic cortical rhythms, and movement-related cortical potentials. Clinical fellows will have the opportunity to attend the movement disorders clinic, perform detailed electrophysiological assessment of patients with movement disorders, and train in electromyography and botulinum toxin injections. |

Description: | Sunnybrook Health Sciences Centre, an academic teaching hospital fully affiliated with the University of Toronto, is offering a one year fellowship position in Stroke Neurology (with the option to extend to 2 years). Sunnybrook has a multidisciplinary stroke program with strong collaboration from neuroradiology, vascular neurosurgery and neurocritical care. Our centre has a bi-plane neurointerventional angiography suite, two 1.5T and one 3.0T clinical MRI scanners (as well as additional dedicated research scanners), 4 CT scanners and PET-CT. We have a dedicated NeuroDoppler ultrasound laboratory which is the only Intersocietal Accreditation Commission Vascular Accreditation (ICAVL) accredited laboratory in Toronto for both carotid US and transcranial Doppler US. We have an active research program and participate in a number of high profile clinical trials in both acute stroke management and stroke prevention. Sunnybrook also houses the Sunnybrook Research Institute and has expertise in clinical epidemiology and stroke health services research. Our team consists of 6 dedicated stroke physicians (Drs. Description: | The Stroke & Cerebrovascular Medicine Fellowship Program at the St. Michael’s Hospital's Comprehensive Stroke Center has availability for one to two stroke fellows per year. The goal of the fellowship is to train future neurologists who can offer highly specialized care to patients with cerebrovascular diseases, especially ischemic and hemorrhagic stroke. Core training will include stroke inpatient and outpatient evidence-based-management, acute and hyper-acute stroke care of patients in the emergency room and critical care units, along with opportunities to participate in electives: Interventional neuroradiology, neuroradiology, Trauma-Neurocritical ICU, Neuro-ophthalmology, CNS demyelination clinic, Rehabilitation rotation, and Carotid US lab. Opportunity to spend some time in community hospitals with stroke units may also be arranged with our partners in the GTA area. Research participation is encouraged. St. Michael's Hospital and Li Ka Shing Research Institute have exceptional research opportunities available. Led by the director of stroke research, Dr. Gustavo Saposnik, who is the Editor in Chief of World Stroke Academy and among the board of directors of World Stroke Organization, the stroke fellowship at SMH and University of Toronto will allow the trainee to get exposure to world-class research. Areas of high impact include equity in stroke care, stroke outcome research, stroke economics, AI in stroke, brain-heart interaction, stroke neuroimaging, quality improvement projects in stroke, stroke robotics among other avenues to get exposure in state of the art research initiatives. The SMH stroke program aims to create future leaders in stroke. The trainees will have the opportunity to get exposure and training in development of protocols, stroke systems of care models, understanding equity in stroke care, basics of health economics, business and operations planning and other avenues that impact patient care in stroke programs. The trainee will have the opportunity to work with various faculty including Atif Zafar, MD who is the medical director of the stroke program at SMH. This is a busy fellowship where fellows will be expected to supervise residents on service. Inhouse alteplase and EVT attendance is required as part of training for fellows during and after hours when on call (fellows take home calls), among other clinical duties. Description: | Stroke-Neurovascular Fellowship Opportunity University Health Network Stroke Program The University Health Network (UHN) Stroke Program is offering one funded Stroke-Neurovascular fellowship position. UHN is a multi-site academic teaching hospital network, that houses one of Ontario’s Regional Stroke Centres at the Toronto Western Hospital (TWH) site. We provide comprehensive clinical stroke care, including hyperacute care with thrombolysis and endovascular thrombectomy, inpatient stroke unit care, and specialized Stroke Prevention Services. We also have an active clinical stroke research program with participation in many national and international clinical trials in acute stroke and secondary stroke prevention therapy. Our Team includes six stroke neurologists, five nurse practitioners and one stroke-clinic nurse. There is strong clinical and research collaboration with Interventional Neuroradiology, Diagnostic Neuroradiology and Vascular Neurosurgery. Our Neurovascular Unit (NVU), a provincially-designated stroke unit, is a 20-bed ward that includes Level 2 (high-acuity) and regular ward care for patients with stroke and other neurovascular conditions. It is managed by a multidisciplinary team consisting of stroke neurologists, vascular neurosurgeons, interventional neuroradiologists, nurse practitioners, and a dedicated interprofessional allied health team, as well as medical students, residents, and fellows. We have specialized outpatient services. Our TIA and Minor Stroke Unit (TAMS) is a dedicated 7-day a week rapid assessment day-unit designed to provide access to same-day clinical evaluation, investigations, and allied health assessment for patients with high-risk TIA and minor stroke. Other stroke prevention services include the Stroke Prevention Clinic for secondary stroke prevention (~3,000 visits a year), and subspecialized, multidisciplinary outpatient care including: Stroke in Young Adults (SiYA) Program, Combined CNS Vasculitis Clinic, Stroke in Women Clinic, the Pediatric Transition in Stroke Clinic, Adult Moyamoya Clinic, and the Neurovascular Clinic. FELLOWSHIP PROGRAM: We offer 1-year clinical (Stroke Expert) and 2-year combined clinical and research (Stroke Scholar) fellowships that align with the National Stroke Fellowship of the Canadian Stroke Consortium (CSC). Certification will be granted nationally through the CSC and through the University of Toronto upon successful completion of the fellowship. Clinical care: Fellows in our UHN-TWH Stroke-Neurovascular Fellowship Program will participate in the emergency assessment and treatment of patients with acute stroke and TIA, in inpatient stroke management, in outpatient care in the Stroke Prevention Clinic and in subspecialized Stroke/Neurovascular clinics of interest. There are also opportunities for participation in electives provided across the stroke care spectrum such as in Thrombosis, Stroke Rehabilitation, and Pediatric Stroke. Description: | This fellowship training opportunity aims to provide physicians with specialized clinical training in providing high-quality palliative and supportive care for adolescents and young adults (AYA), aged 15 to 39 years, with advanced cancer. Existing evidence suggests that AYAs with cancer have unique psychosocial (removed comma) and palliative care needs that differ from those of pediatric and older adult patients. Description: | The purpose of the Palliative Medicine Clinical fellowship is to train physicians to be competent in practicing palliative care at a consultant level. Description: | Toronto Rehabilitation has a long track record in providing outpatient, interdisciplinary team-based rehabilitation to persons with Multiple Sclerosis suffering from mild to moderate levels of impairment. In conjunction with this, there exists a large, and still growing, neuro-physiatric practice catering to the bio-psycho-social needs of MS patients at all levels of disability. Equipped with a dual clinical and research nurse practitioner and MS neuro-physiatrist, this setting is ideal for anyone interested in increasing his or her knowledge of the holistic management of MS patients to minimize disability progression, optimize functional independence and decrease symptom burden. Motivated, caring and dependable specialist physicians would be most welcome to become part of a team dedicated to the well-being of MS patients. No doubt, participation in this setting will enhance one's knowledge of MS specific impairments (such as neurogenic fatigue, MS-related sleep disorders, etc), MS spasticity and pain management, MS neurogenic bladder and more. State of the art information on MS disease modifying treatments, disease surveillance, and exercise and wellness topics will be discussed through the course of this fellowship module. This exclusively outpatient-based practice would complement the acquisition of knowledge, skills and attitudes relevant to the rehabilitation of other brain and spinal cord based pathologies and may form part of a larger neuro-based rehabilitation fellowship for those candidates interested in broader exposure. |

Description: | The Toronto Adult Cystic Fibrosis (CF) Centre is the largest adult CF Clinic in North America and is located at St Michael's Hospital. Care is provided to ~ 480 adults in 3 ambulatory clinics per week and a 15-bed inpatient Respirology unit with over 200 CF admissions per year. Training at the Adult CF Centre will provide extensive experience in the management of CF and its complications (including management of complex, multi-drug resistant infections, massive hemoptysis, pneumothorax, CF-related diabetes, biliary cirrhosis, malnutrition, DIOS, end stage lung disease, patients with multiple drug allergies and management of pregnancy in CF). Fellows will participate in formal Transition clinics held at SickKids, have an opportunity to participate in satellite CF clinics in northern Ontario, and attend Telehealth clinics to see patients living in remote areas. The majority of CF patients listed for lung transplantation in Toronto are cared for at St Michael's Hospital and this provides experience in the transplant referral process, how to care for severely ill patients and in end of life care. The CF Clinics in Toronto are unique in having a comprehensive patient database. This database can be used for quality improvement and research projects. Fellowship Description The Interstitial Lung Disease fellowship at the University of Toronto is a 1-year clinical research fellowship offered by the Interstitial Lung Diseases (ILD) clinic at the Toronto General Hospital. The fellowship will be offered to one individual per year who has finished 2 years of Respirology training in a program accredited by the Royal College of Physicians and Surgeons of Canada. 2. Goals and Objectives of the Training The goal of the fellowship is to provide a trainee with supplementary training, education and research opportunities to advance knowledge in assessment and management of patients with ILD. 3. The Training Structure The ILD fellowship is primarily a research fellowship. Trainee time will be divided as 80% research activities and 20% clinical activities. Trainees will attend multi-disciplinary case conference rounds once per week, radiology rounds once per week, Research in Progress rounds twice per month and will attend our weekly ILD clinic. Trainees will be expected to present cases at our formal pathology MDD once every 4 weeks. During clinical activities, trainees will provide direct patient care under the supervision of the attending physicians in the clinic. Other, subspecialty clinics are optional and may be attended once or twice per month. There is no inpatient component to the Fellowship and there are no “on-call” responsibilities. Non-clinical time (4 days per week) will be spent pursuing research activities. 4. The Application Process The fellowship is administered through the Interstitial Lung Diseases Program at the Toronto General Hospital. Interviews will be held in November and the recipient of the fellowship position will be announced in December for a position starting on July 1st of the following year. Once a person has been accepted into the ILD Fellowship Program, a formal appointment will be initiated by the Supervisor with the Department of Medicine, Division of Respirology, Administration office, and the Post Graduate Medical Education Office (PGME) 5. Eligibility This one-year fellowship accepts a maximum of one individual each year. It is offered to individuals who are eligible to take the written and oral examinations in Respirology at the Royal College of Physicians and Surgeons of Canada. Preference for acceptance to the fellowship will be given to individuals who have demonstrated academic potential with research experience and who have a commitment to an academic research career. 6. Funding Opportunities Trainees will receive a minimum of $60,000 per year given as a bimonthly direct deposit. Description: | The nontuberculous mycobacteria (NTM) Research Fellowship is a one-year fellowship, focusing on pulmonary NTM research, based at the Toronto Western Hospital (TWH) site of the University Health Network. Description: | The University Health Network (UHN) Pulmonary Hypertension Program, located at the Toronto General Hospital, is one of the largest published programs in the country. The program follows over 1000 patients, and provides expert, multi-disciplinary care to patients with all forms of pulmonary hypertension. The fellow will be involved in clinical, educational and research activities of the program. The research team includes clinical fellows, research assistants, clinical trials nurses, clinical epidemiologists and basic scientists. The pulmonary hypertension fellowship allows individuals the opportunity to develop the skills necessary to evaluate and treat patients with pulmonary hypertension and the tools required to become experts in this disease. Additionally they are provided the opportunity to complete a research project and enhance their knowledge of scientific methods and develop a sophisticated understanding of the disease Pulmonary hypertension is a condition that can occur secondary to many pulmonary, cardiac or systemic conditions. It may also occur in isolation - idiopathic pulmonary arterial hypertension. The complexities of the disease, and availability of advanced medical and surgical treatments demands that clinicians receive training in centers with adequate patient volumes and comprehensive clinical and research programs. Established in 1998, the pulmonary hypertension program at UHN is an integrated interprofessional program that strives to set the standard in caring for patients with pulmonary hypertension. As the largest single center Pulmonary Hypertension program in Canada, fellows may conduct research in the full breadth of pulmonary vascular diseases including congenital heart disease, diastolic dysfunction, chronic thrombo-embolic pulmonary hypertension, porto-pulmonary hypertension, HIV, idiopathic and connective tissue disease associated pulmonary hypertension. Fellows will have the opportunity to work closely with the Toronto Lung Transplant Program, and an interdisciplinary environment complements the research experience with nurse educators, cardiology, rheumatology, psychiatry and palliative care. |

Description: | The SHS/UHN Fellowship in Airways Disease provides $70,000 in salary support for national or international trainees to undertake advanced clinical training and academic development in Airways Disease. The one-year fellowship includes outpatient clinics in asthma and airways disease across SHS and UHN sites as well as a breadth of clinical (eg. pulmonary rehabilitation, bronchiectasis, advanced symptom management), research and quality improvement opportunities that can be customized to the recipient’s goals and interests. Approximately 50% of time will be allocated to clinical activities and 50% to scholarly activities. Applicants can be internal (University of Toronto) or external, national or international. Priority is given to Canadian applicants. Description: | Brief Description of the Fellowship Program The Rheumatology fellowship mirrors the training program for our Royal College stream rheumatology PGY4s and PGY5s. The IFTs have the same clinical and research experiences as our Royal College trainees. The PGY4 and 5 years in rheumatology are described in detail below. There is a mixture of ambulatory and in-patient consult rotations that have graduated responsibility over the course of the two-year fellowship program. The PGY4 year is comprised of mandatory rotations meant largely to build a strong general rheumatology foundation. The PGY 5 year offers trainees some ability to customize their year based on their interest but mandates rotations in each of the main sub-specialty rheumatology areas, pediatric rheumatology and a community rotation. Clinical or Clinical/Research Expectations of the Fellowship (% of time that is protected clinical time): This is a clinical fellowship and outside of ½ day per week of research time, the remainder of the fellowship is clinical. The clinical experience for our PGY4’s and 5’s is graduated. In the PGY4 year the emphasis is on General Rheumatology and the acquisition of skills of the specialty, as well as familiarity with the more common rheumatic diseases. In the PGY5 year, the specialty clinics are attended with a view to learning more detail about rheumatic disease entities and extending the skills for detailed evaluation and follow-up. 1) Inpatient Ward: Toronto Western Hospital offers a unique opportunity for trainees to manage rheumatology inpatients directly in the capacity of a “junior staff person”, rounding with residents and teaching them, while at the same time having the supervisory backing of an attending staff rheumatologist. 2) Inpatient Consultation Service: At UHN, the consultation service at Toronto Western Hospital is combined with the inpatient ward service at Toronto Western Hospital. This consists of four elective beds on ward 4B Fell as well as emergency admissions under the Rheumatology service. The remainder of the consultations are done by a second team covering Mount Sinai Hospital in-patients, emergency and ICU, Toronto General Hospital in-patients, emergency and ICU, and Princess Margaret Hospital in-patients. PGY4’s also rotate through St. Michael’s Hospital and Sunnybrook Health Sciences Centre, each of which offers a different type of inpatient consultation experience. In-patients on the consultation services cover the whole spectrum from gout and osteoarthritic knees to pulmonary hemorrhage from ANCA associated vasculitis and organic brain syndrome from Lupus Cerebritis. In all cases the PGY4 takes a supervisory role, working with core internal medicine trainees from the PGY2 and PGY3 levels, doing many cases hands on as well. 3) General Rheumatology Clinics: All participating hospitals offer general rheumatology clinics with opportunity for trainees to work up new and follow-up cases, dictate consultation notes and arrange follow-up under the supervision of teaching staff. All cases are reviewed, dictations are often reviewed with trainees, and teaching occurs during the clinics. Physical examination techniques are observed and Residents have an opportunity to review case synthesis and treatment plan with staff. Joint injections are taught and supervised as part and parcel of this inpatient and clinical consultation experience. General rheumatology is also offered in community clinics, and each resident, gets a community experience somewhere in the Greater Toronto Area (in either their PGY4 or PGY5 year). 4) Specialty Clinics: PGY5 trainees are able to learn rheumatology in greater depth through subspecialty clinics in Ankylosing Spondylitis, Rheumatoid Arthritis, Scleroderma, Systemic Lupus Erythematosus, Psoriatic Arthritis, Vasculitis and Sjogren’s Syndrome. In these clinics residents consider measures of disease activity, radiological and clinical measures of disease damage and construct of clinical research cohorts. These clinics encourage a deeper understanding, greater familiarity and more intense academic curiosity about rheumatic diseases. They have opportunity for intense clinical experience in each of these areas, as well as opportunity to participate in research as per their choosing, fill in research protocols, learn about newer and upcoming therapies, discuss the ethics of clinical trials and take responsibility for management of these complicated cases. All Residents in the PGY5 year also have longitudinal clinics where they follow their own patients for a year. They play a junior staff role as first back-up for call after hours, and they participate in teaching. The PGY4 and PGY5 rotations are reinforced with teaching rounds, half day back, journal clubs and special seminars. 5) Special Opportunities: Opportunities exist university hospital wide for experience with other specialties as requested by the Resident in the PGY5 year. Residents have been given rotations in the past with osteoporosis clinics, sports medicine and nerve conduction and EMG testing. In addition, there are opportunities to work with other specialties in our clinics. The Sjogren’s Clinic is an integrated clinic with ENT, Ophthalmology and dentistry with regular meetings and interdisciplinary discussions. We have a combined clinic with nephrology for patients with Lupus Nephritis, and there has been a combined clinic with hematology for treatment of patients with antiphospholipid antibody syndrome. The Spondyloarthritis Clinic is run with a physiotherapist in attendance. Other opportunities include Pulmonary Hypertension Clinic, Dermatology/Rheumatology Combined Clinic. These latter clinics are multidisciplinary as well, and offer opportunities for collaboration and cross-specialty learning. Research expectations of the fellowship (% of time that is protected research time): Research and other scholarly activities are mandatory. Resident gets some time longitudinally over the year in the PGY4 and 5 year. This is ½ day per week in most cases. In some cases this could be more but would never exceed a total of 4 months over the course of a two year fellowship period. Residents have an orientation session during the first few months of their PGY4 year where they are introduced to the idea and requirements of a scholarly project. This is a formal session where students are provided with an overview of requirements, examples of types of projects and some suggested time lines. Students are provided with a list of University of Toronto faculty, their scholarly area of interest as well as a list of their ongoing projects. Contact information is also included. The PGY 4 trainees are asked to submit a proposal for their research project after the first 3-4 months of their training which has been signed off by their selected supervisor. This project can be related to basic or epidemiologic research, Patient Quality and Safety, Education of some other area of scholarly pursuit. These projects are submitted to and reviewed by the University of Toronto Rheumatology research committee and the trainees are provided with feedback to ensure that their project is achievable and well-conceived. Regular guidance is provided by preceptor and resident meets with Program Director and Division Director together later in year to discuss progress and problems. A progress sheet is maintained by the resident to identify status of research. Program provides a statistician to advise regarding research plan and assist in data analysis. Research is presented at an annual academic day (“Ogryzlo Day”) as well at ACR and CRA Annual Scientific Meetings. This is done under supervision and is an educational exercise. Other expectations that the trainees are required to partake in/completed while in the fellowship (ie. rounds, presentations, research, call, etc…): (1) Teaching: During the first few months of the PGY4 year, students receive a teaching session on “how to teach”. This includes some teaching theory as well as practical physical examination teaching where the trainees are observed in a teaching role and given real time feedback by faculty members. With these skills our trainees are asked to provide teaching to junior trainees when on either an in-patient service, consultation service or out-patient rotation. Junior trainees are generally medical students, internal medicine residents or off-service trainees such as from physical medicine and rehab or family practice. This teaching is sometimes informal but on certain rotations is also formal with organized teaching sessions in a classroom type setting. There are a lot of opportunities for teaching undergraduate clinical skills as well. This is not a requirement of the program but it is an opportunity offered to trainees who are interested. If a student does wish to participate in teaching of undergraduate medicine, they are paired with a staff member who can guide them through the requirements, observe their teaching and provide feedback. (2) Case presentation: Students are required to present to each other during our academic half day “case based learning” discussion. (3) Journal Club/rounds: students also present at journal club once per year and are involved in presentations at rheumatology intra-city rounds. (4) On Call: Trainees are also “on call” to the emergency department and consultation services. This is a rotating schedule and is generally 1 evening or weekend day of call per week. Educational opportunities offered to fellows within the program (i.e. local, national, etc…): Our trainees have a ½ day per week of protected educational time. This is held on Friday afternoons and runs from 8am to 12 pm. There is a pre-specified two-year curriculum of topics that are covered over this period of time. A variety of teaching methods are used including case based discussions, didactic teaching and small group teaching. The topics covered over the two-year period help ensure that all Royal College Objectives are covered over the course of the two-year training program. Every 4 months there is an exam that the students take to evaluate them on the material covered in the preceding months. We also run, in addition to our academic half day, an immunology course. This is a 12-week course taught by rheumatology faculty that covers the basics of immunology and the clinical application of this material. There is an immunology exam at the completion of this course. Our trainees also participate in the Patient Quality and Safety Course offered through the Department of Medicine. This provides them with training in Quality and Safety and together, in the PGY5 year, they complete a Quality project to be presented at the end of the PGY5 year. The American College of Rheumatology provides a Fellowship in Training grant to pay for travel to and registration for the annual scientific meeting in the United States. The Canadian Rheumatology Association pays for travel, accommodation and registration at their Annual Conference. Residents are encouraged to prepare their research for presentation at these conferences. The Ontario Rheumatology Association sponsors an Ontario Rheumatology Resident’s weekend in Toronto yearly. In addition, a National Rheumatology Resident’s Weekend is held every two years, usually in Ottawa. These latter two events are more for the purpose of resident education, formative OSCE examinations and establishment of resident cohesiveness and morale. Outline the scholarly expectations to be undertaken within the fellowship: As per the Royal College requirements, our trainees are expected to complete a scholarly project. The details of this were previously set out under the research section and this is repeated here. Research and other scholarly activities are mandatory. Resident gets some time longitudinally over the year in the PGY4 and PGY5 years. This is ½ day per week in most cases. Residents have an orientation session during the first few months of their PGY4 year where they are introduced to the idea and requirements of a scholarly project. This is a formal session where students are provided with an overview of requirements, examples of types of projects and some suggested time lines. Students are provided with a list of University of Toronto faculty, their scholarly area of interest as well as a list of their ongoing projects. Contact information is also included. The PGY 4 trainees are asked to submit a proposal for their research project after the first 3-4 months of their training which has been signed off by their selected supervisor. This project can be related to basic or epidemiologic research, quality, Education of some other area of scholarly pursuit. These projects are submitted to and reviewed by the University of Toronto Rheumatology research committee and the trainees are provided with feedback to ensure that their project is achievable and well-conceived. Regular guidance is provided by preceptor and resident meets with Program Director and Division Director together later in year to discuss progress and problems. A progress sheet is maintained by the resident to identify status of research. Program provides a statistician to advise regarding research plan and assist in data analysis. Research is presented at an annual academic day (“Ogryzlo Day”) as well at ACR and CRA Annual Scientific Meetings. This is done under supervision and is an educational exercise. Assessments: The program uses a web-based evaluation system, POWER. The supervisors and residents receive the objectives at the beginning of each rotation. A documented assessment of resident performance is completed at the end of every rotation, with mid-rotation feedback occurring at or around the midpoint of every rotation. Residents indicate on-line whether evaluations have been discussed when receiving the ITER. The Program Director supervises these looking for any trends of omission by staff. Other types of evaluations are: ITERs, NWRITE exam, intermittent quizzes, OSCEs, Case Logs, Clinic evaluations, presentations, scholarly project evaluation. Marks are provided after all quizzes and NWRITE exams along with discussion of answers. OSCE evaluations are followed by feedback on an individual and group basisPGY4 trainees meet with Program Director three times in year, PGY5 trainees meet with program director twice in year to discuss progress, ITER’s, program committee evaluation, career planning. |

Description: | Women’s College hospital psoriatic disease program focuses on research of the transition from psoriasis to psoriatic arthritis. Musculoskeletal ultrasound is used as a point of care in the psoriatic arthritis clinic as well as for research purposes. Clinical and research fellowship programs are offered to physicians who completed their specialty training in rheumatology and wish to get additional specialized training in psoriatic arthritis or musculoskeletal ultrasound for a minimum of 12 months. Prior basic training in musculoskeletal ultrasound is preferred for those interested in musculoskeletal ultrasound fellowship. Participation in advanced graduate studies for MSc and PhD degrees is optional. Description: | The goal of the Scleroderma Clinical Research fellowship is to gain advanced expertise in both clinical and research skills in the scleroderma spectrum of disorders. The fellows attend core clinics with Drs. Sindhu Johnson and Zareen Ahmad at Mount Sinai Hospital (MSH) and Toronto Western Hospitals (TWH) longitudinally over 1 year. Fellows are expected to undertake a scholarly project and particpate in observational studies and/or clinical trials of novel therapeutic agents. Fellows critically appraise and present research articles under the supervision of Dr. Zareen Ahmad at the monthly Scleroderma Journal Club. Fellows particpate in the teaching of junior trainees and allied health professionals. Fellows partipate in knowledge translation activities and advocacy activities with scleroderma specific organizations. The fellow attends Scleroderma clinic with Dr. Johnson (MSH, TWH), Scleroderma Clinic with Dr. Zareen Ahmad (MSH), Multidisciplinary Morphea Clinic with Dr. Zareen Ahmad (MSH), Pediatric Morphea Clinic with Dr. Ron Laxer (Hospital for Sick Children). Elective rotations may include Lupus Clinic, Vasculitis Clinic, Rheumatoid Arthritis Clinic, Metabolic Bone Disease Clinic and Pre-Clinical Autoimmunity Clinic. Fellows are expected to review the chart in advance of the patient encounter; order and follow-up on appropriate investigations; identify and manage medical issues; and search the literature when investigational or management options are not clear. Fellows are expected to work collaboratively with staff, allied health professionals and other specialists in the patient’s circle of care. Occasionally, fellows are expected to provide inpatient SSc consultation on the diagnosis and/or management of advanced SSc at MSH, TWH, Toronto General Hospital, and Hospital for Sick Children. Fellows are expected to participate in a minimum of 1 scholarly project from start to finish: framing the question, literature search and synthesis, study design, protocol development, data abstraction form development, research ethics approval, data collection, data cleaning, analysis, interpretation, manuscript preparation, presentation of findings and peer-review publication. Fellows are expected to present their work at the Division of Rheumatology Research Day (Ogryzlo Day), submit an abstract for a national meeting (usually the Canadian Rheumatology Association Annual Meeting), submit an abstract to an international meeting (usually the American College of Rheumatology or European League Against Rheumatism meetings or the Scleroderma World Congress), and produce draft 1 of a completed manuscript. |

Description: | The aim of this fellowship is to gain advanced training in the Systemic Autoimmune Rheumatic Diseases. The core rotations of Scleroderma Clinic, Lupus Clinic and Pre-Clinical Autoimmunity Clinic will occur longitudinally over 1 year. These are longitudinal observational clinics where fellows will gain clinical expertise and be exposed to observational research and clinical trials of advanced therapeutics. Fellows will have the opportunity to undertake one or more scholarly projects that may fall within the spectrum of basic science, translational studies, and observational studies. The trainee will undertake the clinical fellowship in the vasculitis clinic center at Mount Sinai Hospital under the supervision and guidance of senior faculty mentors (Drs.
