Description: | Established in 2006 by Dr. Herbert Ho Ping Kong and funded by G. Raymond Chang, O.C., O.J., this fellowship program supports clinicians from the West Indies (the islands of the former British West Indies) interested in pursuing academic Internal Medicine subspecialty training at UofT. This clinical fellowship is a limited term academic experience, with the objective of providing advanced training and specialized expertise in Internal Medicine subspecialties including, but not limited to Cardiology, Rheumatology, Nephrology, Infectious Diseases, and Gastroenterology or in a closely related discipline such as Neurology. This training is not currently available in the West Indies and such clinical experience is increasingly needed in the tertiary care hospitals around the Caribbean. Upon completion of the fellowship training, physicians will return to their primary institution. The specific goals and objectives would vary depending on the subspecialty.

Description: | The University Health Network (UHN)’s Toronto Congenital Cardiac Centre for Adults (TCCCA), one of the oldest and largest centres of its kind in the world, has trained more Adult Congenital Heart Disease (ACHD) physicians than any other centre in the world (more than 60 physicians since 1993). The TCCCA offers a 2-year fellowship with a broad education in the clinical management of patients with ACHD. There are special clinics for pulmonary hypertension, pregnancy, heart failure and coronary artery anomalies and Marfan clinics. As part of the clinical duties, ACHD Fellows will be involved with inpatient and outpatient care, case conferences, and teaching rounds. ACHD Fellows will also conduct supervised, independent clinical research with a goal of one to two research publications in peer-reviewed journals by the end of the fellowship. |

Description: | The University Health Network (UHN)’s Toronto Congenital Cardiac Centre for Adults (TCCCA), one of the oldest and largest centres of its kind in the world, has trained more Adult Congenital Heart Disease (ACHD) physicians than any other centre in the world (more than 60 physicians since 1993). The TCCCA offers a 2-year fellowship with a broad education in the clinical management of patients with ACHD. The structural / interventional fellow will be expected to attend structural/interventional clinics, review imaging studies for the structural heart disease patient referrals in preparation for their outpatient consultation, attend and be primary operator for congenital, structural, TAVI and Mitral Valve procedures , responsible for the acute care needs of the structural heart disease service including taking inpatient call from home and telephone consultations with outpatients. As part of the clinical duties, fellows will be involved in on-call (in-patient general cardiology ward and ACHD on-call), echo reading, MRI/ cardiac CT, case conference, and teaching rounds. Besides, ACHD Fellows will attend and participate in case conferences, academic teaching rounds, and inpatient review rounds. Furthermore, ACHD Fellows will also conduct supervised, independent clinical research and collaborate in the research activities of the ACHD with a goal of one to two research publications in respected peer-reviewed journals by the end of the fellowship. |

Description: | The Adult Echocardiography fellowship provides formal training in advanced echocardiography (transesophageal, stress (exercise/dobutamine), three-dimensional echocardiography, strain imaging, intraprocedural echocardiography). The Echocardiography Laboratory at the University Health Network is a high-volume center (>25000 cases per year) and a quartenary care facility with expertise in complex cardiac conditions (eg. advanced heart failure and cardiomyopathies, valvular heart lesions, adult congenital heart disease). Research opportunities during the fellowship period include investigations involving novel echocardiographic techniques, the role of echocardiography in the management of cardiomyopathies and valvular lesions, and assessment of educational and quality methods in echocardiography. The Echocardiography fellowship is the only fellowship at UHN that meets level 3 standards for training in Echocardiography, as outlined by current guidelines issued by the Canadian and American Societies of Echocardiography. |

Description: | Duration 1-2 years. Opportunities for a more dedicated research fellowship. Basic and clinical echocardiographic research opportunities, including small animal echo, contrast echocardiography, clinical echo projects. Total 7 echocardiographers 10 sonographers 7 echo rooms, one dedicated stress echo room, and one echo research room, 11,000-12,000 transthorascic (TEE) studies, 350-500 transesophageal (TEE) studies, and 1,000 stress echoes per year. No formal intraoperative TEE program (CV anesthesia). Clinical training will encompass all aspects of echocardiography to include contrast echo, ASD and PFO closures, transcatheter aortic valve implantation, mitral clips, echocardiographic guidance of electrophysiology procedures, 3D echo, non-complex adult congenital heart disease, as well as opportunity to participate in intraoperative echocardiography. Ample opportunity will also be available to participate in basic science and clinical research studies. Collaboration with other imaging modalities, such as cardiac MRI are also available, Upon completion of the Fellowship training program, the candidate will be Level III certified. |

Description: | The Advanced Heart Failure and Transplantation fellowship is an intensive training program for clinical fellows who plan to have a major career focus on caring for patients with advanced heart failure, cardiac transplantation and mechanical circulatory support. There is a mix of outpatient subspecialty clinics, cath lab exposure for hemodynamic assessment, as well as an active inpatient service that includes consultations for advanced therapies, and management of admitted heart failure, transplant and long term left ventricular assist device patients. The program is supported by an academic faculty that provides a formalized educational structure and many opportunities for research/academic productivity. |
