Description:	Goals: To prepare the fellow for a career of leadership in Emergency Medicine at local, regional, provincial, national and international levels. Responsibilities/Expectations: Clinical component: The clinical EM component consisting of twelve (12) to fourteen (14) clinical emergency department (ED) shifts (8 hours each) per month, including at least one (1) full weekend (Saturday and Sunday) and two (2) night shifts per month. The Fellows role in the ED will be that of senior trainee, responsible for patient care, supervision of junior trainees, and patient / department flow, while working under the supervision of a consultant emergency medicine specialist. Responsibility will be commensurate with your background and demonstrated abilities, with progressively increasing roles and responsibilities such that you are capable of functioning independently as a consultant in a high-volume, high-acuity academic ED upon completion of your Program. Six months of clinical work will be carried out at the Sunnybrook and Women’s College Health Sciences Centre Emergency Department (Sunnybrook Site), and six months at the St-Michael’s Hospital Emergency Department (Bond St Site). Both sites are tertiary care hospitals and the only two trauma units for the city, and the main trauma referral centers for the region. They are the core training sites for Royal College Emergency Medicine residents. St-Michael’s Hospital sees a downtown urban patient population with an emphasis on inner city health issues (homelessness, drug abuse, poverty), toxicology, trauma and cardio-vascular disease. The Sunnybrook site sees an urban patient population with an emphasis on trauma, burns, cardio-vascular disease, oncology, geriatrics and women’s health. Clinical education component: in addition to bedside teaching of residents, fellows will be expected to take part in one dedicated teaching shift per month with medical students. Clinical skills including history taking and physical examination, minor procedures (suturing, casting etc), x-ray interpretation etc are emphasized. All Fellows will also have the opportunity to participate in paramedic training and evaluation. Formal educational events: Fellows are expected to attend weekly Division Emergency Medicine Grand Rounds, every Tuesday from 1000-1200, and weekly Hospital EM Rounds at either Sunnybrook or St-Michael’s Hospital. Fellows are also expected to attend rounds specific to their Fellowship Track (e.g. Trauma, Research or EMS rounds). Fellows may be asked to present at rounds from time to time throughout the year. Conferences/CME events: Fellows are encouraged to attend other Emergency Medicine conferences or Continuing Medical Education events when their schedule permits. Research fellows will be expected to submit abstracts based on their work for presentation at scientific conferences. Journal Club: Fellows should participate in monthly Journal Clubs (second Tuesday evening of each month). Eligibility: The candidate must have completed RCPS or CCFP-EM training in Emergency Medicine, or the equivalent training in their country of origin (as approved by the Ontario College of Physicians and Surgeons), be eligible for or have passed certification examinations in Emergency Medicine, and must meet the qualifications for an educational license. Fellows accepted into the program must be board certified in Emergency Medicine in his or her country of origin, prior to the start of their fellowship. The successful applicant will function as a clinical fellow in the emergency department of one or more University of Toronto affiliated hospitals.

Description:	Goals: To prepare the fellow for a career of leadership in emergency medical services at local, regional, provincial, national and international levels. Objectives: Knowledge and understanding of: The historical and legislative framework surrounding modern EMS systems in Canada and the US. The key components and design options for an EMS system, including volunteer and first responder systems. Medical control and various models Medical dispatch and call screening EMS Equipment/Vehicles, extrication and rescue Medico-legal considerations of EMS Approach to high-risk patients Understand legislation and guidelines for training and certification for all levels of providers Understand the different patient care capabilities and responsibilities for all levels of paramedics. Knowledge of the principles of crisis intervention and stress management Guidelines for triage and transport to specialty centers Advantages/limitations of aeromedical vs. land based transport. The physiology of aeromedical transport Aircraft safety and special medical problems Mass gatherings/Disaster Management Funding arrangements for EMS systems Community Involvement and public education Paramedic Education EMS Research. EMS residents should understand principles and special problems inherent in conducting research in the pre-hospital environment, and would be expected to take part in at least one research project during their fellowship. Supervisor: The Fellow will be supervised by the Director of Air or Land EMS programs. Responsibilities/Expectations: The Fellow would develop a curriculum and schedule in conjunction with their supervisor. His or her duties would include: Regular online medical control via the “patch” phone Participation in paramedic training, testing and evaluation Quality control of EMS care The development of pre-hospital protocols Liaison between paramedic and physicians attendance at meetings with government, hospital and EMS personnel Develop an understanding of EMS information systems Development of a research project in conjunction with the supervisor Eligibility: The candidate must have completed RCPS or CCFP-EM training in Emergency Medicine, or the equivalent training in their country of origin (as approved by the Ontario College of Physicians and Surgeons), be eligible for or have passed certification examinations in Emergency Medicine, and must meet the qualifications for an educational license. Description:	Ornge, a not-for-profit organization, is Ontario’s provider of air ambulance and related services. Established in 2006, Ornge is responsible for approximately 21,000 patient-related transports each year, serving a population of more than 13 million people across 1 million square kilometres. With a fleet that includes airplanes, helicopters and land ambulances operating from 12 bases across the province, Ornge crews perform emergent and urgent inter-facility transports and emergent on-scene responses while supporting healthcare in remote communities across Ontario. The Ornge team consists of paramedics, pilots, aircraft maintenance engineers, communications officers, physicians, educators and support staff. The Fellowship will provide a unique experience specifically focused on air medical and land critical care transport. The Fellowship will include an immersive clinical experience in critical care transport in both an urban helicopter emergency medical services environment and fixed-wing opportunities in resource-challenged settings in rural and remote locations throughout northern Ontario. The hospital-based experiences may also include trauma team leader experience at Canada’s busiest trauma centres in Toronto. The successful applicant will be a member of a physician-paramedic team caring for some of the most severely ill and injured patients in Ontario. The clinical care will take place in the transport environment and resource-limited settings, with challenges not typically present in the traditional hospital setting. The Fellowship program will also provide the successful candidate with training and experience to work in Ornge’s Operations Control Centre as a member of the Ornge Transport Medicine Physician Team. Successful candidates will have completed training in Emergency Medicine, Anesthesia or Critical Care Medicine in a program accredited by the Royal College of Physicians and Surgeons of Canada or international equivalent, or Emergency Medicine in a program accredited by the College of Family Physicians of Canada. International applicants with relevant training and experience are also strongly encouraged to apply. The successful candidate must be legally entitled to work in Canada, and hold an educational license with the College of Physicians and Surgeons of Ontario to practice medicine in Ontario
