Description: | Toronto Rehabilitation has a long track record in providing outpatient, interdisciplinary team-based rehabilitation to persons with Multiple Sclerosis suffering from mild to moderate levels of impairment. In conjunction with this, there exists a large, and still growing, neuro-physiatric practice catering to the bio-psycho-social needs of MS patients at all levels of disability. Equipped with a dual clinical and research nurse practitioner and MS neuro-physiatrist, this setting is ideal for anyone interested in increasing his or her knowledge of the holistic management of MS patients to minimize disability progression, optimize functional independence and decrease symptom burden. Motivated, caring and dependable specialist physicians would be most welcome to become part of a team dedicated to the well-being of MS patients. No doubt, participation in this setting will enhance one's knowledge of MS specific impairments (such as neurogenic fatigue, MS-related sleep disorders, etc), MS spasticity and pain management, MS neurogenic bladder and more. State of the art information on MS disease modifying treatments, disease surveillance, and exercise and wellness topics will be discussed through the course of this fellowship module. This exclusively outpatient-based practice would complement the acquisition of knowledge, skills and attitudes relevant to the rehabilitation of other brain and spinal cord based pathologies and may form part of a larger neuro-based rehabilitation fellowship for those candidates interested in broader exposure. |

Description: | The goal of this fellowship is to advance the knowledge and skills required for procedural management of patients who have experienced significant neuromusculoskeletal trauma. Through both inpatient work with sub-acute trauma patients in a rehab centre, as well as work in the outpatient neuromuscular clinic, the fellow will be exposed to a variety of complex orthopedic, soft tissue, and peripheral nerve injuries. Clinical duties will involve in-depth patient assessments that focus on injury and pain management, optimal rehabilitative strategies for best functional outcomes, identification and prognostication of associated peripheral nerve injuries, and physiatric procedural techniques using ultrasound as a clinical tool. Description: | The Toronto Adult Cystic Fibrosis (CF) Centre is the largest adult CF Clinic in North America and is located at St Michael's Hospital. Care is provided to ~ 480 adults in 3 ambulatory clinics per week and a 15-bed inpatient Respirology unit with over 200 CF admissions per year. Training at the Adult CF Centre will provide extensive experience in the management of CF and its complications (including management of complex, multi-drug resistant infections, massive hemoptysis, pneumothorax, CF-related diabetes, biliary cirrhosis, malnutrition, DIOS, end stage lung disease, patients with multiple drug allergies and management of pregnancy in CF). Fellows will participate in formal Transition clinics held at SickKids, have an opportunity to participate in satellite CF clinics in northern Ontario, and attend Telehealth clinics to see patients living in remote areas. The majority of CF patients listed for lung transplantation in Toronto are cared for at St Michael's Hospital and this provides experience in the transplant referral process, how to care for severely ill patients and in end of life care. The CF Clinics in Toronto are unique in having a comprehensive patient database. This database can be used for quality improvement and research projects. There are currently no fellowship positions funded by the Toronto Adult CF program. Cystic Fibrosis Canada offers a one year Clinical Fellowship for physicians who are eligible to work in Canada after their training with the express purpose of preparing future CF clinicians. Description: | Fellowship Description Medical pleuroscopy, airway stent placements, and techniques for management of central airway obstruction using ablative modalities are currently not a focus in this clinical fellowship. Fellows are offered the opportunity to do up to a total of 2 months of clinical electives in another institution with an interventional pulmonology program to gain further exposure and training in these procedures.
