Description: | The Advanced Heart Failure and Transplantation fellowship is an intensive training program for clinical fellows who plan to have a major career focus on caring for patients with advanced heart failure, cardiac transplantation and mechanical circulatory support. There is a mix of outpatient subspecialty clinics, cath lab exposure for hemodynamic assessment, as well as an active inpatient service that includes consultations for advanced therapies, and management of admitted heart failure, transplant and long term left ventricular assist device patients. The program is supported by an academic faculty that provides a formalized educational structure and many opportunities for research/academic productivity. Description: | The Department of Medicine at Women’s College Hospital invites applications for our Fellowship in Ambulatory Cardiology. This Fellowship is open to applicants with completed specialty training in Internal Medicine and Cardiology. The program is intended to provide individuals trained in the fields of cardiology with an opportunity to acquire advanced skills and expertise in the field of ambulatory cardiology including non-invasive imaging modalities with a view to improving the quality of care and education in the ambulatory setting. The training will be supervised by a qualified member of the WCH Department of Medicine Division of Cardiology faculty. QUALITY IMPROVEMENT (QI) STREAM: The fellow will be expected to complete advanced-level training in QI (e.g. Master’s degree at the Institute for Health Policy Management and Evaluation or Veterans Affairs Scholars Program at U of T). Completion of a project suitable for peer-review publication is strongly recommended. RESEARCH STREAM: The fellow will be expected to complete advanced-level training (e.g. Master’s degree with potential for PhD level training or post-doctoral training) in clinical or educational research, with a view to assuming an academic faculty position at a teaching hospital as a Clinician Scientist/Investigator or Clinician Educator, respectively. Fellowship applications to the Quality and Research Streams for longer than one year will be considered. Successful candidates would be required to apply for external funding. Fellows will be expected to be involved in all aspects of arrhythmia-related procedures.
It is not expected that fellows will perform all the above tasks for every patient. However, some cases should be followed through in all aspects from start to finish, while involvement with other cases will only be partial. The clinical care of arrhythmia patients will be shared between the EP fellow and staff physicians, but it is important for fellows to be aware that arrhythmia training extends outside the EP lab or OR. Fellows will be expected to scrub in 150-200 ICD/Pacing/EP cases per year. In accordance with the University of Toronto Division of Cardiology policy, EP fellow (along with other sub-specialty fellowship trainees) may be asked to cover general cardiology in-house call for up to two nights per month. It is anticipated that clinical responsibilities will comprise about 50-60% of the fellows’ time.
RESEARCH The research experience in this combined clinical and research fellowship will be viewed as an essential component, with up to 40% of time allotted to research activities. Both expectations of, and support for, research productivity can be anticipated. Early in the fellowship (within the first four weeks), fellows should conceive (with input from staff) at least one independent research project which they will design and carry out over the year. Submission of one or more abstracts to CCS and AHA (in the spring) will be strongly encouraged.
In the interest of completing the project, the goals should be realistic, and the project should take advantage of existing expertise and infrastructure. Fellows must also be familiar with ongoing EP research protocols and should help in recruitment of patients to clinical trials. Depending on interest and career objectives, the fellow should also consider becoming involved as a collaborator or research assistant on an additional, established project. ROUNDS AND TEACHING Fellows will be responsible in presenting cases at Cardiology non-invasive and Grand Rounds. At least one presentation will be expected during the year at either the Research Rounds (0800 Thursday). On a very occasional basis, fellows may be asked to conduct teaching rounds with the housestaff in the CCU or on the cardiology ward. Didactic teaching will be provided to interventional fellows in accordance with 1999 guidelines of the American College of Cardiology. It is anticipated that these sessions will be run on a “city-wide” basis and include trainees from the other University of Toronto hospitals which have EP services. REMUNERATION There are no funds provided by the University, the hospital, or the Department of Medicine for fellows’ salaries. Prospective fellows are strongly urged to pursue independent sources of funding. However, the Division of Cardiology has agreed to support the specialized training of highly qualified candidates. The amount of support provided will be negotiated, and will depend on the amount of alternate funding available. Fellows will be provided with a pager, lab coats and scrub suits, a comfortable workspace (shared office), and access to a computer (with e-mail and internet access). Financial support to attend at least one major conference per year will also be provided. Vacation allowance consists of three weeks per year. EVALUATION Fellows will meet with the Medical Director of Arrhythmia Service every three months, with both parties providing oral feedback. A written evaluation (ITER) will be completed after 6 and 12 months. On completion of the fellowship program, a certificate will be issued by the Hospital and the University. In addition to scheduled evaluations, ongoing, informal, bi-directional evaluation is strongly encouraged. |

Description: | The Schulich Heart Program is pleased to offer a one-year multi-modality clinical imaging fellowship. The major focus of fellow will be echocardiography, with the goal of achieving a high level of clinical expertise in all applications of this modality. Qualification for Level III certification in Echocardiography, and completion of the National Board of Echocardiography examination, which takes place in July each year, are worthy goals. In addition there will be exposure to cardiac MRI and CT. Primary supervision of the imaging fellow will be by Drs. Joyner, Ahmed, Hansen, Poon and Roifman (echocardiography) and Dr. Anna Zavodni- Medical Imaging (MRI and CT). Drs. Gilbert, Morgan, Poon and Lau will also contribute to supervision of echocardiography. Primary supervision of research will be by Dr. Graham Wright. It is not expected that fellow be involved with every task for every patient. However, it is the joint responsibility of the attending staff, imaging technologists, and fellow to ensure that educational opportunities involving interesting, rare, or unusual cases not be missed. Fellows will be expected to be involved with interpretation and/or scanning of a large volume of echo studies daily, including all interesting/rare/unusual cases that pass through the lab. The fellow will be expected to take part regularly in supervision and interpretation of exercise stress echo. The fellow should be involved in most TEEs that pass through the lab. It is expected that the imaging fellow may be asked to help occasionally with clinical care of CCU or Cardiology ward patients. The fellow may be asked to participate in a TEE on call schedule shared with Anesthesia. No other call responsibility is mandated, although the option of after-hours involvement with interesting cases is encouraged if it is felt to be of genuine educational value, and not an onerous burden. Research: There is the option of being involved in a research project depending on the needs and interest of the fellow. The supervisor for this component of the fellowship is Dr. Graham Wright, research director for the Schulich Heart Program. There is the potential to spend one day per week on research. Teaching is a key mandate of the Sunnybrook non-invasive lab, and an important component of the cardiac MRI and CT programs. The fellow is expected both to benefit from, and contribute to, an excellent teaching environment. Cooperation with core cardiology trainees, elective trainees, and the other Imaging Fellow– especially in situations of limited educational resources – is essential in maintaining a productive atmosphere. Teaching of cardiology residents is an important mission of the non-invasive cardiac laboratories and the fellow is expected to assist in this process. “To teach is to learn” – and it is hoped that the fellow will contribute to this mission. In return, the fellow will receive regular, formal and informal teaching in all aspects of clinical imaging and in conjunction with research efforts. The fellow will be responsible for presenting and leading non-invasive imaging rounds on a regular basis. Presentation at cardiology grand rounds and cardiology research rounds is strongly encouraged. Presentation of educational and quality-assurance rounds within the Echocardiography Lab is also a key component of the fellowship. Description: | The Cardiac Prevention fellowship program at St. Michael's Hospital is a 12-24 month fellowship providing training in all the essential aspects of contemporary preventive cardiology with an emphasis on risk reduction strategies in the patient with coronary disease. Ambulatory care will be emphasised. As well, there is the opportunitiy to explore postmenopausal women's health and aspects of gender equtity. By the end of the training period, it is expected that trainees will have acquired the knowledge, experience, and clinical and technical skills required to be an expert in the diagnosis and management of the entire spectrum of cardiac prevention. GIM training is a prerequisite. The fellowship will be tailored to the trainees background (GIM vs Cardiology training) and needs. In addition to clinical expertise, trainees will also have the opportunity to pursue academic or scholarly activity in the area of cardiac prevention with appropriate supervision at a level required to further a career in academic cardiology, GIM, and/or cardiac prevention. |

Description: | During the clinical echo fellowship year at the Mount Sinai Hospital, there will be a focus on both clinical education and research. The goal will be to achieve level three training in echocardiography. This will result in the ability to direct an echocardiographic Laboratory, train others, and conduct advanced research in echocardiography. This will involve exposure and advanced training in transthoracic echo, transesophageal echo, and 3D imaging, tissue doppler, stress echo both dobutamine and exercise, and strain imaging. With regards to stress echocardiography, the echo fellow’s role will involve patient selection, selection of the stress modality, supervision of the stress tests, as well as integration of the data obtained to prepare a comprehensive report. Competency in contrast echocardiography will involve both rest and stress imaging, as well as the advanced knowledge of administrative protocols, fiscal principals, and specific machine settings to optimize quality. The level 3 echo fellow will be involved in the study of myocardial mechanics and strain imaging, and this involves image acquisition as well as image processing. The fellows will also be aware of the appropriate use criteria in the utilization of echocardiography. The competencies in these various areas of echocardiography’s will be further developed from an educational standpoint. They will be involved in development of case logbooks, conference and case presentations, and training portfolios. They will also have an opportunity to participate in a longitudinal research project during their training year. |
